Duly completed claim form with a description of loss event (attached)

Copy of Civil ID
Statement of account (highlighting unauthorized transaction(s)
Official letter from bank stating the below information:
Date & time card holder reported the incident to Card Issuer (bank)
Total unauthorized charges for which Cardholder is held responsible
Confirmation that claimed transactions are fraudulent and not authorized by card holder
Declaration about any recovery achieved or that unauthorized charges are not recoverable from
third parties as of the date of issuing the letter including undertaking to notify AIG about any
possible future recovery.
