Dear Ms Wujdan Ibrahim,
The Regulations of Legal Competence and Conservatorship (Procedures and Execution) 1970 say:
16. Central Registration of Legally Incompetent Persons
a. An office for the Central Registration of Legally Incompetent Persons shall be maintained near the
Jerusalem District Court, and every person shall be permitted to inspect it upon request.
b. Once a court decreed that a person is legally incompetent, it shall notice the office of Central
Registration of Legally Incompetent Persons; the notice shall indicate the name, ID number, and
address of the person, who was decreed legally incompetent, the date of the decree, the court file
number, and the court that entered the said decree.

Review of previous correspondence, linked below, [1-4] in the matter in reference indicates:
a. Provisions of the Regulations, copied above, regarding the operation of an office for the Central
Registration of Legally Incompetent Persons near the Jerusalem District Court are not complied
with.
b. Provisions of the Regulations, copied above, regarding the permission of every person to inspect
the Central Registration are not complied with.
c. In response to my previous requests, I received letter, which do not carry a reference number, and
their validity as official government correspondence is dubious.
d. In response to my previous requests, I did not receive any printout from the electronic database,
but letters, based on phone calls with the respective court.
Therefore, I request that you provide me a printout from the Legally Incompetent Central
Registration database system, which shows the registration, or lack thereof, of my mother, referenced
above, in the database of the office for the Central Registration of Legally Incompetent Persons.
I also request that such printout be sent to me with an accompanying letter, carrying a reference
number, as is required in official government correspondence, signed by the duly authorized person,
certifying that the printout is from the database of the office for the Central Registration of Legally
Incompetent Persons.
