I hereby swear to take good care of this book as long as it is in my possession. I will not vandalize the book/s nor
tamper the call number attached. I will help preserve this book, by doing the necessary repairs such as gluing/taping
loose-leave/s and/or covering it with clear plastic cover. I will return it, on or before the LAST DAY of FINAL
EXAMINATIONS, intact and in exact same condition as when I had received it, or better. I will return the book/s through a
Student Services Representative, in Student Government Office (SPS 302).
If I fail to return book/s that I have borrowed on or before the scheduled LAST DAY of FINAL EXAMINATIONS, I agree to
have my student ID regulated by the Discipline Office. I also agree to pay a fine of P50 per day late including weekends
and holidays except during the Institutional Christmas Break as announced by the University, Holy Thursday, Good
Friday, and Black Saturday. Said fine shall be paid at the Accounting Office under the account name Lost and Found
Locker and Pahiram Libro Fines Depository Account, upon assessment by the Lost and Found Section of the Discipline
Office. I agree that my student ID will only be cleared upon my returning of all borrowed books to the Lost and Found
section of the Discipline Office (SJ104) and upon full payment of the fine.
If I have damaged the book/s or lost it, I will replace it with a book of the same title and edition (or a later one) in good
condition. Replaced books must be given on the last day of final examinations as well. Late replacements will also be
subject to the P50 per day late fine.
