Credit Card Declaration Letter

GTCM makes it a top priority to prevent credit card fraud and to protect our
customers from unsolicited use of their credit cards.
As part of the procedure to avoid fraud, any client who uses their credit card to fund
their account must provide us with a signed approval of these transactions. This is
standard procedure and applies to all clients, regardless of whether or not they have
completed this form in the past for separate credit cards and/or transactions.
Please see the table below and verify that you are indeed the person who made the
deposit.
Please print this email, sign it and return it to us via fax or alternatively, you can scan
the signed print out and attach it to the reply email.
