Parental Consent Form

All participants under the age of 18 must provide a signed Parental Consent Form in
order to play Paintball &/or Go Karts at Spitfire Indoor Paintball & Go Karts.

I ___________________________ as parent/guardian understand that;

1) Indoor Paintball and Go Karting are physically and mentally intense sports,
2) Indoor Paintball and Go Karting can cause harm to persons not complying
with the rules and regulations provided by Spitfire Indoor Paintball & Go
Karts staff,
3) Injury/harm could occur to my son/daughter,
4) Paintballs travel at speeds up to 280 feet per second and can cause bruising.
I agree my son/daughter;
Is aware of the risks involved with playing Indoor Paintball & Go
Karts,
Is physically fit and able to participate in Indoor Paintball & Go Karts,
Will comply with Spitfire Indoor Paintball & Go Karts rules and
regulations and not use equipment provided in any way other than
instructed by staff members,
Is 12 years of age or over.
I understand all the terms and conditions stated on this Parental Consent Form and
give permission for my son/daughter; ___________________________ to participate
