I, FRANKLIN ANDRADE , of legal age, Filipino Citizen, married and a

resident of No.14 Jade Garden Compound Ortigas San Juan, after being sworn in
accordance with law hereby depose and state that:

I was entrusted by my boss of the warranty certificate of a car battery which is

described as follows:
Battey type: N40 /ISNF
Brand: Outlast Super Premium, 20 months warranty
Serial no. Code: 12396 C/082905;

2. Last January 16, 2006 I just found out that the said warranty certificate was
loss in the cabinet of my boss where it is kept, I do not know how it was loss
either the same was only misplaced by my boss or the same was thrown in the
garbage;
3. I have executed this Affidavit to attest to the truth of all the foregoing facts and
to satisfy the inquiry or requirements of any person or office particularly to
enable me to claim a new battery and for other legal purpose this may serve.
