Tutorial-3

1) Hazard Identification is the first step in all risk assessment or QRA study.
2) If the hazard evolution shows low probability and minimum consequences, then the system is called
GOLD PLATED SYSTEM.
3) HAZOP STUDY identifies potential hazards and operability problems due to deviation.
4) FMEA is a logical structured process that can help identify potential cause of system failure, such as cause
of initiating events or failure of barrier system.
5) FAULTY TREE ANALYSIS is a most commonly used probabilistic analysis used for hazard identification.
6) Which one of the primary keyword?
a) More
b) Reverse
c) Erode
d) Fluctuation
Ans- c) Erode
7) What is HAZARD?
Ans- d) The likehood of something going wrong.

8) What are the different hazard identification methods?

Ans: Process hazard check list
hazard surveys
Hazop
Safety Review.
9) Explain about hazard control, hazard evaluation and hazard monitoring.
HAZARD CONTROLA hazard control program consists of all steps necessary to protect workers from exposure to a
substance or system, the training and the procedures required to monitor worker exposure and their
health to hazards such as chemicals, materials or substance, or other types of hazards such as noise
and vibration. A written workplace hazard control program should outline which methods are being
used to control the exposure and how these controls will be monitored for effectiveness.
HAZARD EVALUATION -A component of risk evaluation that involves gathering and evaluating data on the
types of health injuries or diseases that may be produced by a chemical and on the conditions of exposure
under which such health effects are produced.
HAZARD MONITORING-It is analysis of equipments in order to avoid the accidents by their usage for long
years
10) What is meant by hazard analysis?
Ans:- Hazard analysis contains identification of undesired events that lead to materialization of a
hazard then also deals with analysis of mechanisms by which these undesired events could occur
further estimate the extent magnitude and likelihood any of any harmful effects.
