Dance Officer Try-out Permission Form- Due by Wednesday, February 3, 2009

I give permission for my daughter to try out for Military Officer for the 2010-2011 school year. I
understand that the expectations for a Diamond Military Officer are above the normal school rules and
line member expectations, and failure for my daughter to abide by these rules and expectations may
result in her dismissal if she is selected as a Military Officer. I realize that there is a larger time
commitment for Military Officers. I understand that if my daughter makes Military Officer she must
attend Officer Camp June 5-8, 2010. She must also be a member of the Diamond Officer period during
the school day for both semesters of the 2010-2011 school year. I realize that there are extra monetary
costs to being a Military Officer such as Officer Camp, Officer supplies, and Officer Squads.
