RE: Accurately Completing Bio-Hazard Forms


It has been brought to my attention that many bio-hazard forms for equipment and parts that are
returned to the Thermo Fisher Scientific (Asheville), LLC and Thermo Fisher Scientific
(Marietta), LLC sites are not being filled out correctly, and/or are not being attached to the parts
or equipment. As part of our Return Authorization (RA) process, we send each customer a packet
that includes a Bio-hazard form to be completed by you and attached to the part or equipment
being returned. It is critically important that the Bio-hazard form be completed accurately and
signed-off by a Supervisor. It is important to remember, that people outside of the laboratory
family will come in contact with the part or equipment being returned (Transportation and
Thermo Fisher Scientific associates) and their life could potentially be impacted by how
accurately and completely this form has been completed. With this being said, I am requesting
that you contact me directly via phone at 828-658-2851 or email at
john.scales@thermofisher.com with any questions regarding the Bio-hazard form. Also, we can
protect our most valuable resource, our associates from any potential risk from handling your
return and also expedite our ability to better serve our customer.
