FOLLOWUP EXAMINATION |
INTERIM HISTORY
Ms. Stein is here to follow up on the results of her MRI of the sacrum and SI joints. It does show
a small disc bulge and annular tear at L5-S1. The coceyx segments are normally aligned without
fracture. There is a small subchondral cyst in the left femoral head, only 6 mm. No arthritic
change associated.
PHYSICAL EXAMINATION
Examination is unchanged.
ASSESSMENT
1 Sacral torsion.
2. Left $capular discomfort.
3. LS-SI annular tear which may explain some of her persistent symptoms.
PLAN
L We did discuss the MRI results and implications as well as the natural history of
discagenic pain and she found that reassuring. I think she has done all the right things
treatment-wise. At this point, I would just recommend that she continue with her aquati
exercise and OMT. She was comfortable with that.
2. Twill|plan to follow up with her on a p.r.n, basis.
‘Twenty minutes of the 25-minute appointment was spent discussing the results of the MRI.
