The first two documents are careplans that were completed during the second semester of my junior year in NUR 3112P - Adult Nursing Science II Practicum: Adult Nursing Science II Practicum allows students to continue to build the knowledge, skills, and attitudes regarding the essential holistic care of adults across the healthcare continuum. Students have the opportunity to provide nursing care to clients with common acute and chronic health problems in primary, secondary, and tertiary settings. This practicum is closely aligned with Adult Nursing Science II and Pathopharmacology II. The last two documents are from the first semester of my senior year in NUR 4113 - Gerontological Concepts and Issues. In the Case Study Assignment: The student will listen to an unfolding case scenario simulation of an elderly client and then complete the questions on the Case Study Assignment utilizing evidence-based practice. The Implement Health Promotion Project: The health promotion project will provide the student an opportunity to provide health promotion or clinical prevention for an older adult. Utilizing the nursing process the student will assess, through an interview, plan, and evaluate nursing care as defined on the rubric
posted on Blackboard. The student will evaluate
the project through reflective writing. I have chosen these four documents to show the growth of critical thinking I have attained using evidence-based nursing practice during my journey at Bon Secours College of Nursing. The first two documents are in a concept map form, they illustrate how different aspects of the client affect the clients care as a whole and how I use all of these aspects in deciding which nursing goal(s) is best. The Gero Case Study allows me to take a patient scenario and use my knowledge to address the patients needs and make a plan of how I would address these needs using evidence based practice. The Gero Project Paper is a reflection on a nursing plan that I had created, which includes an explanation on why/how I chose to implement the nursing plan based on evidence based practice, and how this affected the client outcome. All of the documents are evidence of how I have continuously provided safe, quality care using evidence-based nursing practice.
