BASIC QUALIFICATION
1.
Filipino citizen
2.
High school graduate
3.
Physically and mentally fit
4.
At least eighteen (18) years of age but not more than fifty (50) years old
5.
Has undergone pre-licensing training course
APPLICATION DOCUMENTARY REQUIREMENTS
1.
Duly accomplished SOSIA Application Form with 2x2 ID picture (PNPSF01-2005)
2.
SSS Number - Applicants with no SSS Number shall fill-up SSS Personal Record Form (E-1) and
file the same at any SSS Branch for issuance of SSS Number
3.
Duly accomplished SSS Social Security ID Application Form (E-6)
4.
Birth Certificate
5.
High School Diploma
6.
Training - Any of the following:

Pre-licensing Training Certificate (Authenticated by C, Records Branch)

Basic ROTC/CMT Certificate

Retirement Order - for retired PNP/AFP Personnel

Discharged Order - for former PNP/AFP personnel honorably discharged from the
Service
7.
Clearances:

Neuro-Psychiatric Clearance (validated by PNP Health Service)

Drug Test Clearance

NBI Clearance

PNP DI Clearance

Court Clearance

Barangay Clearance
