Areas of need:

I need to learn how to administer assessments better.

I will become more efficient at writing SOAP notes.
I need to incorporate more OT terminology into my notes.
I need to get in a habit of signing my name at the end of documents.

Goal: I will become more efficient at writing SOAP notes.

Methods for achieving goal:
I will find online case studies and write SOAP notes on these.
I will only include necessary information in each section.
I will time myself for 10 minutes while writing the SOAP note.

Time: I will complete two SOAP notes a month from a case study online by May 5 th. I
will be able to write 5 progress notes within 10 minutes by May 5 th.

Areas of Need:

I need to have a better understanding of certain diagnoses to treat patients to the
best of my ability.
I need to expand on my creativity when implementing treatments as well as ensure
that theyre client centered treatments.
Goal: I will have a better understanding of diagnoses that I dont know as well in the
future.
Methods for achieving goal:
I will read up on past power points.
I will ask my fieldwork supervisor for a chance to be exposed to diagnoses that I
need to expand my knowledge on.
I will research 5 diagnoses that I dont have a lot of knowledge about.
Time: I will have more knowledge about 5 diagnoses that I want to learn more about
by Sept 4th.
