Question 4 ‘A fire broke out in a chemical storage and two workers died and eleven more was injured to varying degree, with three having permanent disability. The accident investigation identified following factors as the major cause of the accident: ‘The design and operation of the chemical storage Absence of emergency response plan Lack of training to the operators involved a. Question 4 The company that you are working has 500 workers and it was decided by the management that majority of the current manufacturing facies will be upgraded to an automated system. with latest technology which include use of robots. As one of the top management team member, you are assigned to manage this project. a. Explain the possible reasons for the company to engage into automation and the use of robots, {5 marks} b. __List any FIVE (5) possible types of effects (positive and negative) of automation on workers. {5 marks] c. List and explain any FIVE (6) safety measures that can be instituted for the automated system, [5 marks] d. To accommodate for increasing market demands, your company has commenced an expansion project adjacent to the current production line within the same complex. It is expected that fifty contract ‘workers will be accessing the complex to complete the expansion project. The existing line will continue production as usual during the expansion. a. As the head of operations for the existina line. you have some concems over the safely, security and quality of your production line. Explain why these three aspects are your main concern [5 marks] b. Explain the steps that you will take to ensure that the safety, security and quality will not be compromised in the premise. The medical officer in charge suggests the possible cause of the sudden rise is workplace stress. Your investigation over the issue shows that the possible causes stress are poorly designed workstation and other factors involving workplace environment. a. Provide any FIVE (5) sources of workplace stress. [5 marks] b. The three stages of human stress response are alarm, resistance and exhaustion Explain these stages giving examples each stage. [6 marks] c. Explain what poor workplace design is and give ways to overcome it [5 marks] d. Give FOUR (4) other factors that may have contributed to workplace stress as mentioned above. [4 marks] [TOTAL: 20 MARKS] Question 4 A large chicken processing factory employs 15 workers specifically to manage the freezers and cold-rooms that stores processed chicken and frozen products. These faailities are very large and busy with 3 forklifts operating 16 hours a day. a. _ Give any FIVE types of PPE that these workers need to be supplied with. [5 marks] b. Exposure to extreme hot and cold temperature can lead to heat-stress and cold- stress. Explain the meaning of these stresses and give one example on how each of these stresses can be avoided [4 marks] Explain how operation of the forklfts within the storage facility above could raise some safety issues for the worker. [5 marks] d. The refrigeration system is supplied with ammonia from a pressurised vessel located adjacent to the storage complex. These vessels are periodically examined to prevent leakage and ruptures to go undetected. Give and briefly describe any THREE (3) types on non-destructive testing for pressure vessels.
