Summary:
During my time at Carehouse Healthcare center, I have accomplished four learning
objectives included in my learning contract. I also have accomplished more objectives other than
those in the contract. I have enhanced my ability to provide effective services to the families and
patients by learning practical skills from the employees. I have gained an overall perspective in
the healthcare field by interpreting knowledge, observing professional personnel; to determine if
working for a healthcare facility is an appropriate career goal for me. I have developed an
appropriate detailed report of the facility and its staff members, with the experiences and
observations I've gained, to the administrator to improve the quality services of their facility. I
have strengthen my interpersonal communication skills by offering effective customer service
for the community. I also have gained knowledge by observing occupational therapists provide
rehabilitative care for the patients. I have gain knowledge from the CNAs and LVNs to see how
general skilled nursing is provided. In my time here I have gained clerical, administrative, and
customer experiences.
In my internship, I have provided customer services, by offering beverages such as
coffee, tea, & hot chocolate for visitors, families, and customers. I acquired inventory and
restocked the cart with coffee and tea. I sanitized tables, carts, and chairs when needed. I assisted
the activities director in giving opinions about a wellness curriculum program for the employees.
I passed out brochures to customers to provide knowledge about Carehouse Healthcare Center. I
assisted other volunteers with their tasks. I helped another intern find where she needed to go to
talk to the staff members. I have assisted in patient care by helping nurses attend to patients and
alerting nurses when they need assistance. I have contributed comfort and care for the families
and visitors of the patients making sure they have chairs to sit in. I have observed nurses and
other staffs providing patient care and made sure that they are providing the quality care. With
these observations, I was able to gather information needed to report my experiences here at
Carehouse Healthcare Center to the administrator. I have provided quality professional assistant
to all the staff members that needed my help. I was able to provide assistance to the speech
therapist by translating to a Vietnamese patient's family member because there were no other
translator around. I have assisted the occupational therapist to provide rehabilitation therapy to a
patient. I was able to assist nurses in relocating patients to wherever they need to go.
A highlight of my internship was to be able to provide help to not just the customers,
families, and visitors but able to assist the staff members and nurses. Other highlights during my
time here consists of getting acquainted with the staff members to help gain knowledge if this is
the right occupation for me. I was able to assist staff members in the activities center as I
relocated patients to the center so they can participate in activities such as mini-bowling, bingo,
and other games. Another highlight was that I was able to assist a physical therapist by pushing a
wheelchair behind as the therapist is assisting the patient in walking just in case the patient gets
exhausted and needs to sit and rest. I was able to offer my opinions to the activities director in
creating a wellness curriculum for the employees. I was able to participate in one of the patient's
birthday by singing together happy birthday and participating in games and other activities.
This internship has better-prepared me for my career in health science field by allowing
me to gain knowledge in rehabilitative therapy, speech therapy, dietary and nutrition needs,
general nursing care, hospice care, medication management, and recreation activities. With this
experience I was able to confirm my interest in the healthcare field by helping others and
providing assistance to other employees.
Carehouse Healthcare Center is trustworthy community that offers many services for
patients. The quality of supervision is great as employees are energetic in doing their jobs and
that the patients and families are happy to come back. This center is well organized and the
security is great. Employees show professionalism and display a positive energy for the patients.
The activities provided are high-quality as patients love to participate and always come back to
the activities center. The work environment has a positive energy as employees are well
mannered and profession. If I were able to work in this center, I would do so as it provides great
services and have highly positive staff members.
Imagine that the Centers for Disease Control and Prevention (CDC) has announced a new,
competitive grant that will fund the planning, implementation, and evaluation of a theory-based
health intervention study aimed at reducing the incidence of obesity among Hispanic women. To
have a chance at being funded and carrying out your intervention study, you must develop a
proposal and submit it to the CDC. The CDC requires you to answer the questions below. By
doing so, you will summarize your proposal of your theory-based health intervention study.
Overall, your answers should convince the CDC reviewers that your proposed intervention is
based on an appropriate health science or public health theory; that your plan to develop the
intervention and carry it out is feasible and sound; and that your design of the study to evaluate
the intervention is rigorous and appropriate. The CDC requires that you format all in-text
citations and your reference list using APA format.
Question 1: What is the theory upon which you will base your intervention? Include the name of
the theory, followed by a citation of theorys original authors (at least one citation):

Citation(s): Research by Aten, Strain, & Gillespie (2008) states, "In response to the lack
of a comprehensive psychotherapy theory, the transtheoretical psychotherapy model was
developed (Prochaska & DiClemente, 1982, 1984, 1986; Prochaska & Norcross, 2007). The
result was an empirically supported (e.g., Bowles, 2006; Park et al., 2003) approach to
psychotherapy that has widespread appeal to clinicians" (para. 1).
Question 2: In a paragraph (maximum one page, double spaced), summarize the basic theory.
Your answer should describe the level of influence(i.e., intrapersonal, interpersonal, or
community) and define the major theoretical constructs. Provide at least two appropriate in-text
citations to convince the funder that you know what you are talking about.
Transtheoretical model is a behavior change theory dealing on an intrapersonal level. It
focuses on the stages of change that an individual have to want to change their health behavior.
The theoretical model construct consists of: Precontemplation, Contemplation, Preparation,
Action, and Maintenance. It requires an individual's ability for self-efficacy to go through with
the changes. According to the article by Suris, del Carmen Trapp, Diclemente, & Cousins
(1998), a person in precontemplation does not consider that he/she has a problem and that he/she
does not plan to change (p. 656). Contemplation is when that individual knows of the problem
but struggle with how to change or finding a solution to become dedicated to change (Suris et al.,
1998). An individual in preparation stage tends to attempt the behavior change but has been
unsuccessful effectively to change (Suris et al., 1998). In the article by Suris et al. (1998)
mentions that a person in the action stage is enthusiastically seeking support to successfully
change their behavior. In the maintenance stage, the individual is working to avoid relapse
strengthen by the accomplishments acquired through the action stage (Suris et al., 1998).
Question 3: What is the name of your theory-based health intervention? Please write your
specific, descriptive title in no more than 20 words:
Obesity Prevention & Control among Hispanic Women
Question 4:State the goal of your theory-based intervention by completing the sentence, below.
The goal of my intervention is to prevent and reduce the prevalence of obesity among Hispanic
women.
Question 5: Choose two intervention strategies (e.g., health education, service provision, policy
change) to use in your program. Develop 1-3 activities for each. For each strategy, write a
paragraph that describes the strategy and the related activity(ies) that you will implement. Be
sure to provide a citation for each strategy.
The first intervention strategy we will implement is the Health-related Community
Service Strategy. To improve the health of the participants, we will have clinical screenings to
monitor. We will make these services accessible for all participants (Katz, O'Connell, Yeh,
Nawaz, Njike, Anderson, Cory, & Dietz, 2005).
The next intervention strategy we will include organizational culture activities. These
activities will allow the participants to partake in physical activities. By creating an accessible
environment for the participants will help with more participatory rates. These settings will result
in a positive change to prevent and control obesity (Katz et al., 2005). With this strategy we will
also add healthier food to the vending machines allowing participants to access healthier snacks
during their time in the program.
Question 6: Write one process objective that you will use in your evaluation. (Use the when,
how much, who, what formula.)
Within two months of the program, the staffs will conduct clinical screenings to a minimum of
75% of the participants in the Obesity Prevention & Control Program.
Question 7:In two sentences, name two activities that you will carry out as part of your process
evaluation concurrently with the implementation of your intervention.
Process evaluation activity 1: For our process evaluation, we will find out if our program is
acceptable and effective by giving out questionnaires.
Process evaluation activity 2: Another way to help with the program's effectiveness is to observe
our participants to makes sure the quality of the program successful.
Question 8:Write one outcome objective that you will use in your evaluation. (Use the when,
how much, who, what formula.)
By May 2016, obesity incidence will be reduced to by 30% among the Hispanic women
participants.
Question 9: In one, clear sentence (no more than 30 words), state the hypothesis that you will
test in your outcome evaluation. Make it a directional hypothesis, and be sure to name your
independent variable, your dependent variable, and how you predict they will be associated.
I predict that participants in cultural activities (IV) will decrease obesity (DV) prevalence.
Question 10: In a brief paragraph (no more than 4 sentences), state specifically how the theory
listed in Question 1 will guide your specific health intervention (i.e., how your intervention is
based on this theory).
The intervention strategy of clinical screenings being accessible for the participants can
help individuals maintain and control their health by monitoring their health status. By
monitoring their health status they can prevent relapse. Another strategy of providing accessible
cultural activities can help individuals to participate and change by incorporating physical
activity into their life. This is part of the Action stage in the Transtheoretical theory because they
are seeking help to effectively change their habits.

Center. I have gained valuable insight into the health industry over the past two months.
Because you gave me the opportunity to work on a variety of tasks, I had the chance to observe
numerous aspects of healthcare, from customer service, patient care, staff assistance, and
supervision of nurses. You and your staff were extremely welcoming and helpful, and offered me
terrific career advice.
This internship has definitely increased my interest in pursuing a career in the health field. I
