ERS HOME SERVICES AGENT ENROLLMENT FORM

Home Services is now available to Elite agents! By enrolling in ERS Home Services, you are now are apart of our
Elite renewable energy and energy efficiency services solution. Additionally, by enrolling in this division you are
now able to begin earning immediate commissions on all ERS Home Services. Note: you must complete
Independent Contractor agreements and W9 Tax form, which will be sent to your email and also located in the
back office. This must be completed before any commission check is paid.
Cost is $100 one time administration fee. Should you have any questions, please contact Teresa Do at
