Prelim PGY 1 Surgery

The submitted application indicates that the applicant has been placed in a preliminary PGY 1 position
in General Surgery. Any future ECFMG sponsorship (beyond the applied-for period) will be contingent upon
the applicant's ability to demonstrate progression in training. Therefore, a signed statement from the
applicant confirming an understanding of the following is required:
(1) any future ECFMG J-1 sponsorship will require successful progression in training
(2) the applicant will not be eligible for sponsorship to engage in a repeat of the
preliminary PGY1 year in General Surgery
