IX.

Prognosis
As I observed and handled patient Mrs. MDL in my 5-day practicum ICU, I
could say that the prognosis of her condition is good, considering the patients
response on all medical and nursing management. In fact her GCS is slightly
increasing to 9-10. Vital signs were stable. On the 10 th day of her confinement, all
of her attending physicians agreed to refer her to rehabilitation.
X. Discharge Planning
Patient will be instructed to take the following plan for discharge:
1. Medications
Explain and give emphasis to the patients relative the importance
of strictly adhering to the prescribed medications. Medications should
be taken regularly as prescribed, on exact dosage, time, & frequency,
making sure that the purpose of medications is fully disclosed by the
health care provider.
2. Environment
Emphasize to the patients relative the importance of rehabilitation
setting as well as the importance of having a safe and clean
environment that is conducive for patients recovery.
3. Treatment
Explain to the patient and relative the purpose of the treatment to
be continued at home or activities that needs to be done. Advice
patients relative to seek medical advised when any untoward situation
arises.
4. Hygiene
Stressed to the patients relative the importance of having proper
hygiene.
5. OPD such as regular follow-up check-ups should be greatly
encouraged to patient as ordered by physician to ensure the continuing
management and treatment.
6. Diet which is prescribed should be followed. Low sodium, low fat diet is
a helpful one.
7. Spirituality. Above all, its essential to keep our faith in God.
