INTRODUCTION This report has been prepared as a summary of work completed by Earth Tech, Inc. (Earth Tech) in support of the Remedial Design and Remedial Action (RD/RA) at the North Cove Landfill Superfund Site located in Toledo, Ohio. This report is prepared in accordance with requirements of the February 5, 2003 Consent Decree, Case No. 96-2141. In accordance with the Consent Decree, the Construction Completion Report includes the following elements: = A synopsis of the work defined in the approved RD/RA Work Plan and the Final Design and certification that this work was performed. = An explanation of any changes to the work defined in the approved RD/RA Work Plan and Final Design, including as-built drawings of the constructed RA facilities, and why the changes were necessary or beneficial for the project. * A brief description of the outstanding construction items for the pre-final inspection and an indication that the items were satisfactorily resolved. * Certification that the constructed RA or component of the RA is operational and functional, TL CONSTRUCTION COMPLETION REPORT 2. SYNOPSIS OF WORK The North Cove Landfill Committee (NCLC) selected Earth Tech, Inc. to perform the RD/RA work for the North Cove Landfill in Toledo, Ohio. The RD/RA Work Plan prepared by Leggette, Brashears & Graham, Inc. (LBG) was approved by the Ohio Environmental Protection Agency (OEPA) in May 2003. The purpose of the RD/RA was to implement the OEPA approved remedy for the North Cove Landfill so that human health and the environment are protected. Specifically, the RD/RA work was intended to reduce the potential health risk associated with dermal contact with impacted soils and waste in a delineated area within the footprint of the former landfill, and along the banks of the Ottawa River and the Bowen Ditch. The scope of work stated in the RD/RA Work Plan consisted of additional soil cover, riverbank and ditch slope revetment, and phytoremediation. Institutional controls consisting of fencing and bollards were also installed for maintenance of the integrity of the remedy. Operation and maintenance (O & M) are also planned to maintain the integrity of the remedy. In addition, deed restrictions were placed on all three properties in 2005. Earth Tech submitted the Final RD/RA Report to the OEPA and NCLC on July 28, 2003. The Pre-Construction Conference with all interested parties was conducted at the site on August 18, 2003. After review, OEPA approved the Final RD/RA Report on August 20, 2003. Full-scale construction activities commenced in late August, 2003. Monthly progress reports were submitted to the OEPA and NCLC during the construction portion of RD/RA work. Copies of these reports have been included in Appendix A and should be referred to for specific details regarding events that occurred during RD/RA construction, RD/RA construction of the soil cover, riverbank and ditch slope revetment, and phytoremediation was conducted according to the Final RD/RA Report and design drawings. However, some issues with establishment of the living systems within the RD/RA were noted after construction and are discussed in Section 4.1. Earth Tech prepared a Construction Quality CONSTRUCTION COMPLETION REPORT Assurance (CQA) Report related to the RD/RA which details elements of the soil cover, Bowen Ditch revetment, fencing and bollard placement, walking path, Ottawa River concrete block revetment and phytoremediation system construction, As-builts of the landfill soil cover construction are included in the CQA report attached as Appendix B. 2.1 OTTAWA RIVER REVETMENT INSTALLATION Work performed on the Ottawa River (revetment construction and phytoremediation) is discussed in this report. Revetment was placed over the defined coverage area along the banks of the Ottawa River as outlined in the Final RD/RA Report. 79,818 square feet of open-cell block revetment material was placed on both the north and south banks of the Ottawa River. ‘Appendix C includes the shop drawings for the mats, and details for the mats. The top edge of the mats was secured using an anchor trench as constructed according to manufacturer's recommendations. Geotextile material was placed in the area above the normal water elevation in order to release seepage coming out of the highway embankment into the river channel due to leachate buildup or a possible rapid drawdown of the water level in the river. After the mats were placed, topsoil was installed in the cores of the open-cell blocks. Due to the slope of the revetment, topsoil was installed using a stone slinger and raked into the cores by hand. 660 cubic yards of topsoil was installed into the cores. The revetment area and disturbed area on the ‘bank above (i.e. anchor trench) was then hydroseeded for phytoremediation. 2.2. PHYTOREMEDIATION INSTALLATION The revetment area and disturbed areas on the bank above (i.e. anchor trench) were hydroseeded with the following seed mix: 35 pounds per acre (Ib/acre) of Pasture Type Tall Fescue Kentucky 31 Certified (Festuca arundinacea Schreb.), 3 Ib/acre of White Clover (Trifolium repens L.), and 15 Ib/acre of short annual rye grass, as per the phytoremediation design. Phytoremediation trees (Populus Deltoides) were then planted on the north slope of the Ottawa River as follows: a total of seventy (70) trees were planted: 1) within the landfill limits (on the east side, alternating between 36” bare root and 36” cuttings on a 10 foot spacing, above the revetment area and below elevation 585 above mean sea level (MSL) as a single row for 200 feet (20 trees) and as two rows for 100 feet (20 trees)); and 2) outside the landfill limits (west of the revetment, alternating between 36” bare root and 36” cuttings on a 20 foot spacing, below elevation 585 above MSL as a single row for 440 feet, using existing trees as available (30 new trees). CONSTRUCTION COMPLETION REPORT As part of the OEPA Section 401 Water Quality Certification, separate from the original phytoremediation design, trees were planted on the south bank of the Ottawa River. These plantings are listed in Section 3.9 of this report. 2.3. SURVIVABILITY OF PHYTOREMEDIATION INSTALLATIONS ‘The phytoremediation installations (grasses and trees) along the banks of the Ottawa River were to be inspected after one year of installation to determine the success of installation and the requirement for repair or re-installation. 75% of the phytoremediation plants were required to be established after one year. A site inspection of the phtyoremediation areas was conducted on November 2, 2005 by Patricia Beaujon (DaimlerChrysler), Troy Simmon (Earth Tech), and ‘Matthew Peak (Earth Tech). The inspection showed the following: = On the south bank of the Ottawa River, both the phytoremediation grasses (Pasture Type Tall Fescue Kentucky 31 Certified and White Clover) have become well established (i.e. ‘greater than 75% established); * On the south bank of the Ottawa River, 29 of the 29 mitigation trees planted (red maples) have become well established (100% established). * On the north bank of the Ottawa River, the Pasture Type Tall Fescue Kentucky 31 Certified has become partly established on only the bottom half of the bank. Overall, Tall Fescue is sparse on the top half of the north bank. Tall Fescue on the north bank is estimated to be 33% established. White Clover has become well established on the entire north bank (ie. greater than 75% established). * On the north bank, of the seventy (70) phytoremediation trees (Populus Deltoides) originally planted, two (2) were identified to have survived (3% established). ‘The mitigation systems are living systems and its establishment is subject to many uncontrollable forces such as temperature and rainfall. The phytoremediation system will be enhanced in the spring of 2006 and will undergo periodic inspections and maintenance for a period of 5 years, after which it should be well established. 24 CONSTRUCTION COMPLETION REPORT Earth Tech is currently evaluating the best method to re-install the portions of the phytoremediation system which did not survive. A plan to re-establish these portions will be submitted under separate cover. 2.4 MONITORING WELL INSTALLATIONS New monitoring wells were installed to monitor groundwater within the landfill footprint. One well was installed upgradient of the landfill, and three wells were installed just west of the Ottawa River, within the landfill footprint. A construction memorandum detailing the well construction was submitted under separate cover and is included as Appendix D of this report, Future monitoring of the groundwater will be detailed in an Operation and Maintenance Manual prepared by others. Any significant changes to the work defined in the approved RD/RA Work Plan and Final Design are discussed in the following section. CONSTRUCTION COMPLETION REPORT 3. CHANGES TO THE RD/RA WORK PLAN AND DESIGN A summary of the changes to the RD/RA Work Plan and Design are listed below in chronological order. A brief description of the change, including reference to as-built drawings of the constructed RA facilities where applicable, and why the changes were necessary or beneficial for the project, are discussed. As-built drawings for all elements of the Final & modified RD/RA are included as an attachment to this report. 3.1 _ EXPANSION OF NON-CONTIGUOUS LANDFILL AREA AND SUBSEQUENT CAPPING During the excavation of waste within the original non-contiguous area, it was discovered that the waste extended outside of the area delineated in the original RD/RA work plan to the north and northwest. The excavation was subsequently expanded in this direction, until it was decided that this method was not cost-effective in determining limits of waste. A Geoprobe unit was mobilized to the site to more efficiently determine the limits of waste, As a result of this investigation, the limits of waste were defined, and the area of non-contiguous waste was determined to be approximately 0.5 acres. After consultation with DaimlerChrysler, it was agreed that consolidation of this material into the main landfill footprint was no longer cost effective. Consequently, this area received the same 2-foot soil cover system as described for the ‘main landfill footprint in the RD/RA. The limits of the expanded non-contiguous area and details regarding the soil cover construction may be found in the attached CQA report. This change was necessary in order to assure the same level of protection to human health and the environment, 3.2 INSTALLATION OF RIP-RAP IN LIEU OF UNIFORM SECTION MAT (USM) REVETMENT IN THE BOWEN DITCH In the approved RD/RA, 4-inch thick USM revetment was proposed in the Bowen Ditch Revetment design. Rock rip-rap was altemately evaluated and placed instead of the USM. It ‘was determined that rip-rap would provide the same level of dermal protection for human health as the USM. A design modification was submitted to OEPA on December 17, 2003. A copy of this comespondence is included as Appendix E. INS 3.3. EXTENSION OF BOWEN DITCH REVETMENT AREA ‘The coverage of the revetment material on the southeast portion of the Bowen Ditch (I-75 embankment side) was extended from the original design to the limits as shown on Drawing 2 of the as-built drawings included with this report. The extension of revetment coverage was based on field borings and laboratory analysis which determined the extent of existing 2-foot soil coverage over the landfill. This change was necessary to ensure an adequate level of dermal protection for human health at the Bowen Ditch. 3.4 EXTENSION OF LANDFILL FOOTPRINT IN NORTHEAST CORNER NEAR DREXEL DRIVE During rough grading of the area just outside the northeast comer of the main landfill footprint, waste was discovered. The extent of the waste outside of the previously designated landfill boundary was defined by test pit activities and the landfill soil cover was extended to include this area of approximately 2,500 square feet. The northeast extension is shown on the as-builts in the attached CQA report. This change was necessary in order to assure the same level of protection. to human health and the environment over the newly discovered waste. 3.5 REQUEST FOR EXTENSION OF CONSTRUCTION SCHEDULE THROUGH OCTOBER 15, 2004 During the initial construction period of the RD/RA, the United States Army Corps of Engineers (USACE) issued a final determination in a letter dated October 23, 2003 that a 404 Permit was not required for the North Cove Landfill site. It was consequently concluded that an OEPA Section 401 Water Quality Certification was not required. However, after further discussions with USACE and OEPA, it was determined that an OEPA Section 401 Water Quality Certification was indeed required. Due to the delay in determination of the need for a 401 Permit, the revetment construction on the Ottawa River was delayed until the 2004 construction season, Due to the delayed construction and in anticipation of the required public comment period, an extension of the construction schedule was formally requested. This change was necessary to assure that the RD/RA construction of revetment on the Ottawa River was conducted in compliance with an OEPA 401 Permit. A completed OEPA 401 Permit application was submitted in May 2004. 32 CONSTRUCTION COMPLETION REPORT 3.6 REQUEST FOR EXTENSION OF CONSTRUCTION SCHEDULE TO DECEMBER 1, 2004 Due to further delays in the receipt of the OEPA Section 401 Water Quality Certification, and resulting future delays in construction, an additional extension of the construction schedule was requested by DaimlerChrysler in a letter dated June 2, 2004. This change was necessary to assure that the RD/RA construction of revetment on the Ottawa River was conducted in compliance with an OEPA 401 Permit and that there was sufficient time to complete the phytoremediation portion of RD/RA construction. 3.7 FENCE/BOLLARD PLAN AND SITE FACILITIES CHANGES The fencing and bollards to be placed as shown in the RD/RA around the main landfill site west of 1-75 were intended to prohibit public vehicular traffic from traversing the landfill footprint. During the course of the RD/RA construction, the fencing and bollard plans were changed to accommodate future use of the site as a city park. A gravel access road was constructed in order to service the Lockwood Regulator at the end of Hillcrest Avenue, within the landfill. The City of Toledo has since paved this gravel access road. The final plan for the fence, bollards and site facilities is shown on the as-builts included in this report, and it should be noted that the final as- built fence/bollard plan achieves the intended goal of preventing unauthorized vehicular access to the site, 3.8 INSTALLATION OF ALL OPEN CELL REVETMENT MATERIAL In the approved RD/RA, revetment material on the Ottawa River was proposed to consist of both ‘open- and closed-block revetment material. It was subsequently proposed that all open-block revetment material be used across the entire extent of the revetment area. This design resulted in a beneficial increase in available habitat in the river for sediment-dwelling benthic organisms while still being effective in preventing dermal contact. The as-built drawings (Drawing 4, 5 and 6) for the Ottawa River revetment construction reflect the change in the revetment material. 3.9 OEPA SECTION 401 WATER QUALITY CERTIFICATION MITIGATION PLANTINGS As part of the OEPA Section 401 Water Quality Certification, separate from the original phytoremediation design, trees were planted on the south bank of the Ottawa River. Twenty- CONSTRUCTION COMPLETION REPORT nine (29) 6-foot tall, Ohio-native species trees (red maple) were planted along the river 100-feet ‘west and 200-feet east of the landfill footprint for mitigation purposes. 3.10 CHANGES MADE BY CITY OF TOLEDO SINCE COMPLETION OF CONSTRUCTION ‘The constructed area of the RD/RA located west of I-75 has been utilized as a City of Toledo Park (Liberty Park). After construction of the RD/RA, a number of additions/changes not listed in the Final RD/RA design report were completed by The City of Toledo, Within the landfill footprint, trees for the park were planted and the gravel access road that was built for servicing the Lockwood Regulator has been paved. In addition, park structures (benches, playground equipment, pavilion, lighting) have been installed in the area outside of the landfill footprint. 34 CONSTRUCTION COMPLETION REPORT 4, PRE-FINAL INSPECTION A pre-final site inspection for construction completion was conducted at the site on November 10, 2004. Present were: Patricia Beaujon (DaimlerChrysler), Paul Jayko (OEPA), David Leffler (City of Toledo), Tom Linkous (ODOT), David Johe (Brown & Caldwell), Troy Simmon (Earth Tech), and Steve Cervi (Earth Tech). OEPA did not cite any non-compliance issues, recognizing that tree plantings on the north bank of the Ottawa River were still required as per the schedule and that some erosion repair work around the site would probably be required in the spring of 2005. Since the time of the meeting, all tree plantings for the phytoremediation portion of the RD/RA have been completed. Erosion repair work was conducted in May 2005. Fencing and guardrail replacement was conducted in the Toledo Jeep plant parking lot (landfill footprint east of the Ottawa River) in August 2005. 4.1 NoTED Issues Since the time of the meeting, periodic inspections of the RD/RA installment have identified issues with the establishment of living systems which require corrective measures. The two issues which have been noted will be addressed in the Spring of 2006. The first issue is regarding erosion of the main swale within the main landfill footprint. Due to heavy precipitation during establishment of the grasses within the swale, erosion of the main swale has occurred, The main swale will be repaired in the Spring of 2006 to ensure the maintenance of the soil cover over the main landfill. The second issue is with the establishment of phtyoremediation grasses and trees on the northwest bank of the Ottawa River due to weather conditions. The phytoremediation issues will be remedied in the Spring of 2006 to ensure the proper operation of the entire phytoremediation system. ‘A repair plan for these identified issues with the establishment of living systems will be submitted at a later date, prior to conducting repairs. 45 CONSTRUCTION COMPLETION REPORT 5. CERTIFICATION OF CONSTRUCTED RA ‘The RD/RA Work Plan consisted of additional soil cover, riverbank and ditch slope revetment, and phytoremediation for purposes of reducing risk to human health and the environment posed by dermal contact. Certification of soil cover thicknesses on the main landfill footprint and the expanded non-contiguous landfill area have been documented in the CQA report in Appendix B. Bowen Ditch revetment installation details, Ottawa River revetment, and phytoremediation plantings occurred as outlined in the Final RD/RA Report and design drawings, and are detailed in the as-built drawings attached to this report. Earth Tech hereby certifies that the constructed RA is operational with the exception of the two issues with the establishment of living systems noted in Section 4.1 and was constructed per the original design and/or modified as described within this report. (Earth Tech) in support of the remedial design and remedial action at the North Cove Landfill Superfund Site located in Toledo, Ohio. This report is prepared in accordance with requirements of the February 5, 2003 Consent Decree, Case No. 96-2141. In accordance to the Consent Decree, the monthly report should include the following stipulated items: the progress of work activities, the percentage of work completed, difficulties encountered, actions taken to rectify problems, work activities planned for next month, changes in key personnel; target and actual completion dates for work activities, schedule including milestones and explanation of any deviations from milestones, any analytical data collected during the reporting period, and identification by media, of any generated, treated, or disposed waste. Approximately 15 percent of the remedial action has been completed at the site. Upcoming task items due to be completed include: * Coordinate preparation of Section 401 Certification application with DaimlerChrysler, Ohio EPA Division of Surface Water and the Army Corps of Engineers = Preparation of monthly newsletter for residents * Completion of soil cover system in landfill and non-contiguous area. Issues Encountered ‘Two issues arose during the previous month which merit discussion: Surface slas/cinder debris. The on-site construction coordinator, Mr. Steve Cervi, noted the presence of slag/cinder at the surface of the ground in several areas outside of the established landfill boundary. Samples of this material were taken for analytical testing, which is included as an attachment to this report. Results of this testing showed concentrations less than VAP Table II Soil Standards for residential areas, with the exception of a slight exceedance of benzo(a)pyrene in one sample, which is consistent with sampling conducted for the final RD/RA. ‘Non-contiguous area expansion. During the excavation of waste within the original non-contiguous area, it was discovered that the waste extended outside of the area delineated in the bid documents, to the north and northwest. ‘The excavation was subsequently expanded in this direction, until it was decided that this method was not cost-effective in determining limits of waste. A Geoprobe unit was mobilized to the site to more efficiently determine the actual limits of waste. As a result of this investigation, the limits of waste were defined, and the area of non-contiguous waste was determined to be approximately 0.5 acres. (Earth Tech) in support of the remedial design and remedial action at the North Cove Landfill Superfund Site located in Toledo, Ohio. This report is prepared in accordance with requirements of the February 5, 2003 Consent Decree, Case No. 96-2141. In accordance to the Consent Decree, the monthly report should include the following stipulated items: the progress of work activities, the percentage of work completed, difficulties encountered, actions taken to rectify problems, work activities planned for next month, changes in key personnel; target and actual completion dates for work activities, schedule including milestones and explanation of any deviations from milestones, any analytical data collected during the reporting period, and identification by media, of any generated, treated, or disposed waste. Site Activities Earth Tech completed the following task items during the July/August 2004 reporting period: * Vegetation clearing activities in preparation for installation of revetment on banks of Ottawa River (in progress); = Rough grading of banks of Ottawa River (in progress) in preparation for revetment placement. Jayko Ohio Environmental Protection Agency ‘August 23, 2004 Planned Site Activities Next Period Earth Tech has planned the following activities for the upcoming reporting period: * Completion of site entrance installations (asphalt tumaround at .Hillerest/bollards/wood fence) (end of August 2004). = “Topsoil installation and hydroseeding along extreme west of property (end of ‘August 2004) * Preparations for installation of revetment on banks of Ottawa River (completion of rough grading, construction of anchor trenches) (August/September 2004) = Installation of revetment on banks of Ottawa River (September 2004) Approximately 60 percent of the remedial action has been completed at the site. Upcoming task items due to be completed include: "= Phytoremediation installation (trees and tall grasses) within revetment on banks of Ottawa River (October 2004). Issues Encountered 401 certification has still not been finalized. As a result, work at the site had to be halted and contractors rescheduled. This also has delayed the planting of the phytoremediation grasses which will affect their germination and growth. Actions Taken to Rectify Issues Potentially include a cool season, short, annual rye as part of the seed mix to help provide soil stability until phytoremediation grasses are established. Schedule The final completion date of the Construction Schedule, in compliance with the Consent Decree, has been extended to Dec. 1, 2004 as requested in DaimlerChrysler’s letter dated June 2, 2004. ‘Wastes Generated/Treated/Disposed None.
