FOREWORD
ThisCodeofPracticeonsurveyanddraftingforminesurveyplansisanapprovedcodeofpractice
undersection274oftheWorkHealthandSafetyAct(theWHSAct).

Anapprovedcodeofpracticeisapracticalguidetoachievingthestandardsofhealth,safetyand
welfarerequiredundertheWHSActandtheWorkHealthandSafetyRegulations(theWHS
Regulations).

Acodeofpracticeappliestoanyonewhohasadutyofcareinthecircumstancesdescribedinthe
code.Inmostcases,followinganapprovedcodeofpracticewouldachievecompliancewiththe
healthandsafetydutiesintheWHSAct,inrelationtothesubjectmatterofthecode.Like
regulations,codesofpracticedealwithparticularissuesanddonotcoverallhazardsorrisks
whichmayarise.Thehealthandsafetydutiesrequiredutyholderstoconsiderallrisksassociated
withwork,notonlythoseforwhichregulationsandcodesofpracticeexist.

CodesofpracticeareadmissibleincourtproceedingsundertheWHSActandRegulations.Courts
mayregardacodeofpracticeasevidenceofwhatisknownaboutahazard,riskorcontroland
mayrelyonthecodeindeterminingwhatisreasonablypracticableinthecircumstancestowhich
thecoderelates.

Theremayberelatedjurisdictionalspecificlegislatedrequirementsforsurveyinganddrafting
plans.Personsshouldinvestigateandidentifythoserequirementsintheirjurisdictionandcomply
withthem,alongsidetheWHSlegislativerequirementsandguidanceprovidedinthisCode.

CompliancewiththeWHSActandRegulationsmaybeachievedbyfollowinganothermethod,
suchasatechnicaloranindustrystandard,ifitprovidesanequivalentorhigherstandardofwork
healthandsafetythanthecode.

Aninspectormayrefertoanapprovedcodeofpracticewhenissuinganimprovementor
prohibitionnotice.

ThisCodehasbeendevelopedbySafeWorkAustraliainconjunctionwiththeNationalMine
SafetyFrameworkSteeringGroupasamodelcodeofpracticeundertheCouncilofAustralian
GovernmentsInterGovernmentalAgreementforRegulatoryandOperationalReformin
OccupationalHealthandSafetyforadoptionbytheCommonwealth,stateandterritory
governments.

AdraftofthisCodewasreleasedforpublicconsultationon[tobecompleted]andwasendorsed
bytheSelectCouncilonWorkplaceRelationson[tobecompleted].

SCOPEANDAPPLICATION
ThisCodeappliestosurveyinganddraftingforthepreparationandreviewofaminesurvey
plan(s)andkeepingacopyforinspectionundertheWHSRegulations.Itprovidesguidancefor
mineoperatorsonhowtoapplytheWHSRegulationsbycoveringthehardcopy,digitalthemes,
minespatialinformationandstorageoftheminesurveyplan.Italsoprovidesguidanceonthe
preparationofotherplans,whichmaybeusedtopreparetheMineSurveyPlan,byaccessingthe
minesstoredsurveydata.

TherequirementsoftheWHSRegulationsforminesurveyplansareintendedtohelpensurethe
safetyoftheminebyaccuratelyidentifyingthelocationandfeaturesofminingoperationsand
infrastructure.OperationalsurveyinganddraftingunrelatedtotherequirementsoftheWHSAct
andRegulationisnotcoveredbythisCode,forexampleengineeringplanspreparedfor
productionandrecoverytocalculatestockpileandexcavationvolumes.

AsstatedintheForeword,theCodedoesnotextendtojurisdictionalspecificlegislative
requirementsforthepreparationofminesurveyplans.Mineoperatorsandcompetentpersons
shouldinvestigatewhetherthereanyfurtherlegislativerequirementsandguidanceintheir
jurisdiction.

WhoshouldusethisCode?
TheprincipalusersofthisCodearemineoperatorsandcompetentpersonsinvolvedinthe
preparationofminesurveyplans.Themineoperatormayusethiscodetoassistinensuringthat
theplansdevelopedbyemployedorcontractsurveyorsmaymeetthemineoperatorslegislative
duty,assetoutinclause675O.

HowtousethisCodeofPractice
ThisCodeincludesreferencestobothmandatoryandnonmandatoryactions.Thereferencesto
legalrequirementscontainedintheWHSActandRegulations(highlightedintextboxesinthis
Code)arenotexhaustiveandareincludedforcontextonly.

Thewordsmust,requiresormandatoryindicatethatlegalrequirementsexistandmustbe
compliedwith.Thewordshouldindicatesarecommendedcourseofaction,whilemayindicates
anoptionalcourseofaction.

Thiscodeshouldbefollowedbymineoperatorsandcompetentpersonsinallmineswhen
preparingminesurveyplans,whichmayinvolvesurveyors,asrequiredinregulation675O(3)
below:

(3)Theplanmustclearlyshowthefollowing:
(a)thesurveyofthemine,includingdisusedsurveyandboreholes;
(b)thelocationofelectricalinstallations;
(c)thelocationoftelephonesandotherfixedplantassociatedwiththeradioand
telecommunicationssystems;
(d)waterdamsandtailingsdams;
(e)naturalfeaturessurroundingthemine;
(f)placesforthestorageofhydrocarbonsorexplosives;
(g)pointsofentryandexit,includingemergencyexits;
(h)refuges(inanundergroundmine).
Note
Regulation634requiresthemineoperatorofanundergroundmine
toprepareaplanoftheventilationsystemforthemine.

AMineSurveyPlanmayconsistofoneormoresheetsshowingtheentiresurveyofamineinplan
orsection.Eachsheetrepresentsapartorwholeofeachmininglevelorseamundergroundoran
areaofopencutorquarryingminingoperationsshowninplan.

ThisCodeshouldbeusedwhencompilingaMineWorkingsPlanforeachminingoperation,so
thattheplansdatamaybeincludedintheMineSurveyPlan.Mineoperatorsarenotrequiredby
clause675O(3)toprepareaMineWorkingsPlan,butshouldfollowtheadviceintheCode
concerningmineworkingssoastoshowitclearlyontheminesurveyplan.Theymayalsofinditof
assistancewhencomplyingwithanylegislativerequirementsformineworkingplansintheir
jurisdiction.TheMineWorkingsPlaninthisCodemayincludeanycrosssectionorlongitudinal
sectionsheetsused.

1.1Preparingminesurveyplans
Whenpreparingminesurveyplans,themineoperatormustensurethecompetentperson
referencesitaccordingtotheWHSRegulationsrequirements:

675O1Ajurisdictionmayomitsubregulation:

Theminingoperationmayoperateonalocalsurveygrid/datum.Howeverallminesurveyplans,
includingitsminespatialinformation,mustbecompiledontheMapGridofAustralia1994
(MGA94),basedontheGeocentricDatumofAustralia1994(GDA94)values,unlessotherwise
allowedforinajurisdictionbyclause675O(3).

Classesofminesinjurisdictionsexcludedfromtherequirementsoftheaboveclause675O(2)
shouldsurveyofftheirlocalgridandprovidetransformationdetails(orsimilar)onthemine
surveyplan.

Arangeofminesurveyingmethodologiesmaybeusedandreferencedtotheabovelegislated
gridsanddatums.Thesemethodologiescaninvolveremotesensingtechnologiessuchas:

photogeometrywithGPScoordinates

airbornesensors,whichcaninvolve:
aerialphotographyandphotogrammetricmapping
aeriallaserscanningandterrestriallaserscanning
orthoaerialphotography

Whateverthemethodologyused,itshouldachieveafinalaccuracywithin2metresinorderto
prepareanaccurateminesurveyplan.Mineoperatorsshoulddeterminebyriskassessmentthe
specificlevelofaccuracytheyrequirefortheirsurveyinganddrafting,havingregardfor
publicationssuchastheICSMSP1(seesection2.3ControlsurveysandSubsidiarySurveysbelow
formoredetails).

Theminesurveyplanshouldbesectionedintosheetsthatbestfitthemine.Indeterminingthe
bestfitforthesheetlayoutthecompetentpersonshouldkeepinmindtheneedtoprovidefor
expansionofthemineandadjoiningminessurveyplans.

Whereoldsurveysexist,thecompetentperson(s)preparingandmarkingtheminesurveyplan
shouldassumetheyconstituteahazardunlessprovenotherwise.Allplansshouldbereferenced
withcautionuntiltheiraccuracyhasbeenverifiedandvalidated.TheWHSRegulationsrequires
themineoperatortoobtainhistoricalminesurveysoftheminetocarryoutthisverificationand
validation:

1.2Symbols
Thetechnicalsymbols,signconventionsanddefinitionsforstrataandrocktypesshownonthe
minesurveyplansandassociatedminespatialinformationshouldbeinaccordancewiththe:

AustralianStandardforMinePlansPreparationandSymbols(AS4368),and

AustralianStandardforGeologicalSymbols(AS2916).

IfasymbolisnotprovidedforinAS4368orAS2916thecompetentpersonshouldcreateasuitable
symboltobeshowninthelegendontheminesurveyplan.

1.3Dutiesofamineoperator
TheWHSRegulationsrequiremineoperatorstoensurethatadetailedsurveyplanofthemineis
preparedbyacompetentpersonandkeptatthemine.ThisCodesetsoutanumberofwaysin
whichmineoperatorsmaydothis,includingengagingpersonsasrequiredunderWHSRegulation
clause675O:

1.5Inspectionofminesurveyplans
TheWHSRegulationsrequirethemineoperatortokeepminesurveyplansandmakethem
availableforinspectionundertheWHSAct:

Plansshouldbekeptinaformatsuitablefortheminesotheycanbeviewedandreadilyaccessible
tosatisfytheabovelegislativerequirements.Forsomeminesthemostappropriateformmaybe
onpaper,whileothersitesmayfindanelectronicformatmoreappropriate.Previousversionsof
retainedplansmayhavebeenpreparedunderdifferentgridsystems.Personsinspectingthem
shouldbemadeawareofthisbydetailsshownontheplanand/orthroughtherecordssystem.

2.1OriginofCoordinates
Allsurveysshouldoriginatefromtheminebaseline,oranyothermarkincludedinthestatesurvey
controlnetworkthathasastandardofaccuracyconsistentwiththeminebaseline.
ThesurveyoftheminebaselineshouldbeplannedandsurveyedtoClassBstandardsof
accuracyasdefinedinICSMSP1.
Note:foranexistingmine,thecurrentbaselinemarksmayberetained,providedtheyareofa
durablenature.

2.2UndergroundBaseline
Eachundergroundminemustestablishundergroundbaselines,insuitablepositionsinthe
undergroundsurveyandaslongaspracticable,soastoprepareaccurateanddetailedmine
surveyplans.Theterminalmarksshouldbestableanddurable.Undergroundbaselinedetails
shouldberecordedonaminesurveyplan.

2.3ControlSurveysandSubsidiarySurveys
Wherenoprescribedaccuracyisdefinedinajurisdiction,eachControlSurveyshouldachievea
standardofaccuracyasprescribedinICSM,SP1toClassDorbetter.Whencalculatingcompliance
toICSMSP1ClassDusingtheformular=c(d+0.2)thisdirectionspecificallydefinesdasthe
following:d=distancetoanystationinkm,withaminimumvalueof1(km).
Eachcontrolsurveystationshouldbeadequatelyreferencedandsubstantiallymarked.Asfaras
practicablethemarksshouldbeplacedinapositionleastlikelytobedisturbed.
Wheredifferentiallevellingisusedtoestablishtheheightofacontrolmark,ICSMSP1ClassLD
shouldbeused.
Subsidiarysurveysshouldbeemployedbythecompetentpersonwherenecessarytowithinafinal
accuracyof2metresofposition.Thisaccuracymaybeachievedusingremotesensingtechniques
andGlobalPositioningSystems(GPS).

2.4CorrelationofSurfaceandUndergroundSurveys
CorrelationbetweensurfaceandundergroundsurveysshouldbeconsistentwithaClassDsurvey
asprescribedinICSMSP1,unlessotherwiseprovidedforinjurisdictionalspecificlegislation.
Incorrelationofsurfaceandundergroundsurveys,wheremethodsotherthandirecttraverseare
employedforazimuthorcoordinatetransfer,thesurveysshouldbeshownonaplanseparate
fromtheminesurveyplanandshoulddisclosethespecialsurveymethodsemployed.Theplan
shouldberetainedattheoperationandthecompetentpersonshouldcertifythatthesurvey
shownontheplanisaccurate.

theminetoensuretheaccuracyoftheminesurveyplan.

675QSurveyplantobeavailableforinspection
(1)Themineoperatorofaminemustkeepthecurrentminesurveyplanandallpreviousversions
oftheplanavailableforinspectionundertheAct.
(2)Themineoperatorofaminemustmakethecurrentsurveyplanreadilyaccessibleonrequest
toworkersatthemine.

Themineoperatormustensurethesafepreservationofallminesurveyrecordssoastosatisfy
theregulationrequirementssetoutabovetoobtainhistoricalminesurveysandmakeprevious
versionsofminesurveyplansavailable.Recordsshouldbekeptatthesurveyoffice,orother
suitablestoragefacility.
Minesurveyrecordsshouldbemaintainedeithermanually(inafieldbookorotherstable
material)orelectronically.Whereamachineorotherdeviceisrequiredtoaccessthestoreddata
themineoperatorshouldensurethedataisregularlyupdatedtoamediaandformatthatis
currentlyavailable.
Minesurveyrecordsshouldbepermanentlyrecordedandmaintainedsotheymaybeutilisedfor
futuresurveys.
Wheresurveysarerecordedinelectronicformtheinformationrecordedshouldbeconsistent
withthatrequiredforsurveybooks.
Allsurveybooksshouldbemaintainedingoodorderandshouldhavethefollowingdescription
clearlyandpermanentlymarkedonthecoverandinsidetitlepage:

thenameofthemine,

forundergroundmines,thelevelnameandcataloguenumbertowhichthebookrefers,
and

consecutiveindexnumber.

Thefollowingproceduresshouldbeadoptedforentriesintosurveybooks:

allsurveyobservationsandmeasurementsshouldberecordedatthetimeofsurvey;

whenamistakeneedstobealteredthereshouldbenoerasure,theerroneousentry
shouldbestruckthroughandthecorrectionwrittenabove

thedatumlineofthesurveyandtheazimuthadoptedshouldbeclearlyindicated

lengthsshouldbeenteredatthetimetheyaremeasuredandwherenecessarycorrections
shouldbeclearlyindicated

bearinganddistancefromreferencemarksshouldbeclearlyshown

referencemarksandbenchmarksplacedbythesurveyorshouldbenotedwiththe
positionsanddescriptionsshownbyasketchintheappropriatebook

linesremeasuredshouldbesospecifiedandoriginaldistancesandbearingsshown,and

thecompetentpersonshouldsignanddatethefieldbookshowingtheworkwas

Acompleteandseparateduplicateofsuchrecordsshouldbepreservedonpaperordiscorother
permanentelectronicmedium.Whereamachineorotherdeviceisrequiredtoaccessthestored

2.6Beforepartsofaminebecomesinaccessible
Beforeanypartofaminebecomesinaccessible,thepositionofthemineworkingsshouldbe
establishedfromacontrolorsubsidiarysurveybycompetentpersons.
Wheremineworkingsareunabletobesurveyed,thecompetentpersonshouldtakeallreasonably
practicablestepstoassistthemineoperatorinthecompletionoftheminesurveyplan.

2.7SurfaceMovementandSubsidence
Surveysshouldbeundertakentorecordsurfacemovement,includingsubsidenceinducedby
mining.
Thesesurveysshouldbecarriedoutunderthesupervisionof,andcertifiedby,thecompetent
person.
Acopyofallsurfacemovementand/orsubsidencesurveydata,includingfieldnotes,shouldbe
retainedbythemineoperator.

2.8SurveyofBoreholes
Boreholesshouldbeidentifiedbyuniquenameornumberandshouldbeshownonthemine
surveyplan.Collarand/orcommencementlocationsofboreholes,shouldbeestablishedfroma
controlorsubsidiarysurvey.Thefollowinginformationshouldbeutilised,whenavailable,from
directsurveyorothersourcetodefinethepositionofaboreholeandsuitablyendorsed(for
exampleadrillerslogorgeophysicalsurvey):

totallength(ordepth);

inclination(ordeclination);and

planprojection(bearingorazimuth)whenotherthannearvertical.

3.1PreparingtheMineSurveyPlan
Theminesurveyplanmustbepreparedby,orunderthesupervisionof,thecompetentpersonas
requiredbyWHSRegulation675O(1).Inaddition,themineoperatormayberequiredby
jurisdictionalspecificlegislationtosubmitcopiesofthePlantotheRegulatoratcertainspecified
times.
Inpreparingtheminesurveyplan,thecompetentpersonshouldcertifythattheyhavepreparedit
tosatisfythelegislativerequirements.See3.8Certificationbelowformoredetails.
Inthecaseofanundergroundmine,theminesurveyplanmustcomprisesheetsforeachseamor
Levelalongwithotherlayoutdataforthewholeofthemine,assetoutinthisCode.
Inthecaseofanopencutmine,theminesurveyplanmustbecompiledontheOpenCutVoid
Sheetsalongwithotherlayoutdataforthewholeofthemine,assetoutinthisCode.
TheLevelSheets,SeamSheetsandVoidSheetsmustbecompiledatascalethatbestreflectsthe
minesurveywithsufficientclarityandnumberofsheetstocovertheextentofthesurveyand
necessaryadjacentsurvey.
Whennewsheetsoftheminesurveyplanarebeingpreparedandanypartofthepreexisting
mineiscompleted,anoutlineofthepreexistingmineworkingsmaybedrawnandthenewplan
sheetendorsed,withreferencetoanypreviouslypreparedminesurveyplansfordetail.
Shouldthepreexistingmineworkingsbecomeactive,orusedforaccessinanareapreviously

appliances.FirefightingappliancesshouldberepresentedbysymbolsinaccordancewithAS
4368.
Theplanshouldshowpositionsinwhichpipemains,hydrants,isolationvalves,firesubstations
andfiredepotsaresituated.Thisplanshouldalsoshowthepositionofventilation
walls/stoppings,trapdoors,regulators,seals,preparedsealsites,overcasts,aircrossings,
ventilationdoors,beltconveyors,mainelectricsupplycables,fixedelectricalapparatus,high
voltagecablesandswitchgear,telephonesandthedirectionofventilationintheroadways.
Theplanshouldbereviewedand,ifnecessary,updatedatnogreaterthanthreemonthlyintervals
andpossiblymorefrequentlydependingontheminingoperations.

5.6.

EmergencyEscapeandRescuePlan(undergroundoperations)

Theescapeandrescuesurveyplanshouldbecompiledgenerallyatascalethatbestreflectsthe
minesurveyandescapeandrescueappliances.Theplanshouldincludeallsurveyofthemineto
enabletheselfescapeand/orrescueofpersonnelfromanywhereinthemine.
Thisplanshouldshowmainandaccessroads,themeansofexitingfromeachpartofthemineto
thesurface,allselfescapesystems,refugechambers,firstaidstations,telephonestations,belt
conveyors,anygasdrainagemainsandbranches,knownfalls,accumulationsofwater,stowage
identifiedasnonpassable.
Thisplanshouldalsoshowanyinformationfromthefirefightingplanandtheventilationplanor
anyadditionalinformationidentifiedasnecessaryforthedealingwithanemergencyatthe
operation.

Theplanmaybemaintainedasspatialinformation.Topographymaybeshownifitaddstothe
interpretationoftheplan.
Inthecaseofanundergroundmine,thesurfaceplanshouldshowallstreets,roads,reservoirs,
swamps,waterbodies,unconsolidatedsurfacedeposits,railways,mainpipelinesandanyother
