    Allhumansubjectsresearchprojectsmustbereviewedandapprovedbeforehumansubjectsarerecruitedor
    involvedinresearchactivities.AllprojectsmustbepresentedonanIRB1Formor,ifappropriate,completion
    ofanApplicationforExemptionForm.

    Applicationswillnotbereviewedbeforetheyarecomplete.

    Thefollowingmaterialsmustbesubmittedinorderfortheapplicationtobecomplete:
    TheIRB1form,completed,signedbytheRPI,andsubmittedeitherelectronicallyorbyhardcopy.
    Thecompletegrantapplication,contract,deliverables,orotherdocumentationoffundingapplicationfor
    externallyfundedresearch.
    Relevantconsentformsandscripts.
    Allmeasures,suchassurveys,interviewprotocols,inventories,anddescriptionsofexperimentsthatare
    notalreadyincludedontheIRB1underProcedures.Draftversionsoftheseitemsareacceptableifthe
    finalversionsarenotavailable.
    RecruitingMaterials.
    Asapplicable,PleasecompletetheResearchTeamAttachmenttolistallindividualsengagedinthe
    researchstudy,includingthosefromotherinstitutions.
    Asapplicable,documentationofpermissionsandapprovalsfromoutsideinstitutionsandtheirIRBs,ora
    statementaboutthestatusoftheseapprovals,ifpending.
    Asapplicable,AppendixA(ResearchEquipmentForm),withattachments.
    Asapplicable,AppendixB(Drugs,Agents,andDevicesForm),withattachments.
    Asapplicable,AppendixC(BiologicalMaterialsForm),withattachments.
