Sixth Grade Science Safety Contract (Adapted from FLINN Scientific)

PURPOSE: Science is a hands-on laboratory class. However, science activities may have potential hazards. We will use
some equipment that may be dangerous if not handled properly. Safety in the science classroom is an important part of
the scientific process. To ensure a safe classroom, a list of rules has been developed and is called the Science Safety
Contract. These rules must be followed at all times. Additional safety instructions will be given for each activity.
No science student will be allowed to participate in science activities until this contract has been signed by both
the student and a parent or guardian. (Signature forms are found on the separate sheet)

1. Conduct yourself in a responsible manner at all times in the science room. Horseplay, practical jokes,
and pranks will not be tolerated.
2. Follow all written and verbal instructions carefully. Ask your teacher questions if you do not understand
the instructions.
3. Do not touch any equipment, supplies, animals, or other materials in the science room without
permission from the teacher.
4. Never eat, drink, chew gum, or taste anything in the science room.
5. Keep hands away from face, eyes, and mouth while using science materials. Wash your hands with soap
and water before leaving the science room.
6. Wear safety glasses or goggles when instructed. If instructed, never remove safety glasses or goggles
during an experiment. There will be no exceptions to this rule!
7. Keep your work area and the science room neat and clean. Bring only your ISN notebooks, lab
worksheets, and writing utensils to the work area.
8. Clean all work areas and equipment at the end of the experiment. After cleaning, return all equipment
to the proper storage area.
9. Follow your teachers instructions to dispose of any waste materials during in an experiment.
10. Report any accident (fire, spill, breakage, etc.) or injury (cut, burn, etc.) to the teacher immediately.
11. Always carry a microscope (or any other heavy materials) with both hands. Hold the arm with one hand;
place the other hand under the base.
12. Never open storage cabinets or enter the prep/storage room without permission from the teacher.
13. Dress properlylong hair must be tied back, no dangling jewelry, and no loose or baggy clothing.
Wear aprons when instructed.
14. Learn where the safety equipment (fire blanket, eye wash station, fire extinguisher, etc) is located and
how to use it. Know where the exits are located and what to do in case of an emergency or fire drill.

I have read and understand the Sixth Grade Science Safety Contract. I understand that these rules must be
followed during each lab activity. I know that if I do not follow these rules that there will be consequences and
I may not be able to complete the lab which will affect my grade.

constant communication (via webpage visits, emails, etc.) with Ms. Flythe to ensure the highest quality of
education for my child. I understand that Ms. Flythe will hold my child to high, fair, and consistent expectations
in her classroom.
I have read and understand the Sixth Grade Science Safety Contract. I understand that my child must follow
these rules during each lab activity. I also understand that if these rules are not followed that my child may not
be able to complete a lab activity, which will affect their grade on that particular assignment.
