On 22.12.204 around 12:02 pm, we conducted walk thru joint inspection with
Engr. Ayman of DAR Safety Engineer together with PreTech Co. Safety Officer related
to safety issues and concerns in the fabrication workarea. We observed some unsafe
practices, workplace condition and work environment that are hazardous and
harmful to direct workers. Listed below were visually found during our inspection.
1. No certification from 3rd party of cranes, equipments, riggers as per safety officer
of Pretech Co.
2. No evacuation plan poster or signage.
3. No access signages i.e. Entry/ Exit
4. Monthly inspection of Fire Extinguishers were not being done
5. Fire hose reel cabinet were obstructed by materials
6. Unsafe single plank were placed as access being used
7. Fabricated rebar around 20mm dia.were being utilized
8. Poor housekeeping inside the work area
9. Damaged electric cables in powered tools
10. Exposed open wires of welding machine
11. Distribution box without locking device
12. Cylinder tank of acetylene without flashback arrestor
13. No proper and safe workarea of hotwork activity
14. No Lock Out/ Tag Out system
15. Scattered protruding nails on the plywood
16. Damaged rigging material (web sling)
17. Unsafe work platform more than 6 ft. high (unguarded and no access)
18. Hook does not have safety lock device (safety latch)
19. Improper placing of garbage bin causing of flies and unwanted odor.
20. Lack of safety prea
cautions posted like signages.

Safety control measures must strictly implemented accordingly. As per safety

officer of Pretech Co.around 200 workers are working in day and night shift
schedules.
