TO REGISTER FOR THE COURSE:

In order to reserve a seat in a course, please complete the registration form and mail it with a $250 nonrefundable registration fee to 872 East 29th Street, Brooklyn, NY 11210. Please note that the $250
registration fee will be applied to the course tuition of $1250 (Includes required equipment: Blood
pressure cuff, Stethoscope, CPR Face Mask and Clinical Rotation Lab Coat.) A seat in the course can
only be guaranteed if the registration fee is received and you attend the first day of class, on time. All
others will be admitted to the class on a first come first serve basis (space permitting). Once your
registration is received, you will receive a letter confirming your seat in the course.
*If you cannot attend the course you registered for, your $250 registration fee can be applied to another
course for up to 6 months at the current tuition rate at that time. Please note that if you do not attend the
first class on the course start date, you may have forfeited your seat in the course.
