This is in reference to the outstanding obligation for the hospitalization of certain Mrs.
ANGELINA P. BEROIN in the amount of FIVE HUNDRED THOUSAND PESOS
(P500,000.00), more or less, which was promised to be paid by the issuance of postdated
check/s within a period of six (6) months commencing on March 2005 until the entire
obligation has been fully satisfied.
We hereby guarantee that the said account of Mrs. ANGELINA P. BEROIN shall be
fully settled at the given period of time, otherwise, the undersigned guarantor would be
compelled to pay the same in her behalf.

HUNDRED THOUSAND PESOS (P500,000.00), more or less, for which we have
requested your goodself to act as the Guarantor in the payment of the same.
In writing your goodself, the undersigned Newspaper Dealer of The Philippine Star
hereby express my gratitude for accepting the said commitment. In return, I hereby
undertake to take full responsibility of the said obligation for which I commit to pay the
same to your goodself in case of its failure of payment with the hospital in due time.
