The undersigned is the lawful owner of that one (1) unit Nissan Sentra (model 2001) with
Plate No. WTH-896, which I caused to insure with this prestigious insurance company
under Policy Number PCNCWY0500053NA issued last December 14, 2005. Due to
change of interest, I have sold the vehicle to Mr. RODOLFO R. JAVIER as attested by
a DEED OF SALE OF MOTOR VEHICLE dated February 2, 2006, which is enclosed
herein for your reference.
In writing your good office, I am requesting for a refund of the insurance coverage fee for
said vehicle in favor of the current owner, Mr. RODOLFO R. JAVIER, as the latter
desires that the insurance coverage of the car be covered by his companys in-house
insurance company.

matter.
