Tuberkulosis primer
Tuberkulosis primer terjadi karena infeksi melalui jalan pernapasan (inhalasi) oleh
Mycobacterium tuberculosis. Biasanya pada anak-anak. Kelainan rontgen dapat berada
dimana saja dalam paru-paru, dan dapat mengenai beberapa segmen dalam satu lobus paru.
Walau begitu, bagian yang sering terkena adalah lobus bawah, lobus media dan lingula, dan
segmen anterior dari lobus atas.3
Manifestasi yang paling sering ditemukan pada tuberkulosis primer adalah
pembesaran kelenjar limfe / limfadenopati. Dengan ditemukannya pembesaran kelenjar limfe
hilus dan mediastinum, dapat dipastikan adanya tuberkulosis primer, karena pada
tuberkulosis post-primer jarang ditemukan kelainan ini. Angka kejadian pembesaran kelenjar
limfe ini semakin menurun seiring dengan bertambahnya usia seseorang.3

Kelainan radiologis yang tampak selain pembesaran kelenjar limfe hilus dan
mediastinum dapat berupa konsolidasi (kelainan berwarna putih) yang dapat berawan,
berbentuk garis (linier), bulat (nodular), menyerupai massa (mass like) maupun konsolidasi
homogen. Kelainan berupa konsolidasi ini sering timbul segmental ataupun lobaris, dan
menurut data statistik kelainan yang didapat lebih sering pada paru sebelah kanan.3
Salah satu komplikasi yang mungkin terjadi adalah pleuritis, yang ditandai dengan
adanya efusi pleura (pada foto akan tampak meniscus sign dan tanda-tanda pendorongan).
Pleuritis terjadi karena perluasan infiltrat primer ke pleura melalui penyebaran secara
hematogen. Komplikasi lain adalah atelektasis akibat stenosis bronkus karena perforasi
kelenjar ke dalam bronkus. Baik pleuritis maupun atelektasis tuberkulosis pada anak-anak
mungkin demikian luas sehingga sarang primer tersembunyi di belakangnya.3

Young male patient with fever and cough has a focal opacity in the left lower lobe that looks like a
pneumonia. This is a case of primary tuberculosis in an adult.4
Posteroanterior chest radiograph in a young patient shows a right upper lobe and right lower lobe
consolidation and a small pleural effusion on the right side. 4
A middle-aged man presents with a cough and fever lasting several weeks. Posteroanterior chest
radiograph shows a prominent paratracheal area on the right, lymphadenopathy, a cavitary opacity in
the right upper lobe, and a focal consolidation in the middle lung zone on the right. The patient was
ultimately found to have primary progressive tuberculosis.4
Tuberkulosis post-primer
Tuberkulosis yang bersifat kronis ini terjadi pada orang dewasa. Saat ini pendapat
umum mengenai penyakit tersebut adalah bahwa timbul reinfeksi pada seorang yang dimasa
kecilnya pernah menderita tuberkulosis primer, tetapi tidak diketahui dan menyembuh
sendiri. Sarang-sarang yang terlihat pada foto Roentgen biasanya berkedudukan di apeks,
segmen posterior lobus atas, dan segmen superior lobus bawah, walaupun kadang-kadang
dapat juga terjadi di lapangan bawah, yang biasanya disertai oleh pleuritis. Dapat juga
ditemukan gambaran adanya kavitas yang merupakan petunjuk atau tanda khas dari
tuberkulosis post-primer. Gambaran kavitas berbentuk bulat dengan dinding atau tepi yang
tipis berwarna putih dan bagian tengah berwarna hitam. Kadang terdapat gambaran air fluid
level di dalam kavitas.3

2. Penyebaran milier
Akibat penyebaran hematogen tampak sarang-sarang sekecil 1 2 mm / sebesar
kepala jarum (milium), tersebar secara merata di kedua belah paru. Pada foto, toraks
tuberkulosis miliaris ini dapat menyerupai gambaran badai kabut (snow storm appearance).
Penyebaran seperti ini juga dapat terjadi ke ginjal, tulang, sendi, selaput otak (meningen),
dsb.
3. Stenosis bronkus
Stenosis bronkus dengan akibat atelektasis lobus atau segmen paru yang bersangkutan, sering
menduduki lobus kanan (sindroma lobus medius).
