Informed Consent

I, __________________, freely and voluntarily consent to be a participant in the research

project entitled Environmental Behavior Study to be conducted at Florida International

University during the 2010 Spring Semester.

I understand that I will not receive any monetary compensation in this study. My

participation will require me to complete two shore surveys and view one video. This

study should be completed within 10 minutes. There are no risks associated with my

participation in this study. Any and all information obtained from me will be kept

completely confidential.

I understand that the purpose of this study is to see if my questionnaire response relate to

material presented during the study. I understand that if any time I feel uncomfortable, I

am free to leave and withdraw my participation. Should I require and more information, I
