The lost IVT Card shall be replaced by the National ANSAP office upon submission of

the following:
1.
Original and photocopy of Certificate of Attendance of the 3-day Basic IV
Therapy training.
2.
Original and Photocopy of Certificate of Training from ANSAP.
3.
Notarized affidavit of loss
4.
Two (2) recent 2X2 pictures with light green background.
5.
Processing fee
6.
Photocopy of PRC License.
Please prepare this before going to PRC
1. Passport size at white background with full name on it
2. Notarized Affidavit of Loss (no need for police report)
3. Paste (adhesive for your picture at the form)
4. 250 pesos for ID duplicate
