This is in regards with my application for an extension of my
break of service.
I am requesting your good office for an approval for my leave of
absence from October 1-15, 2015 due to unavoidable domestic
responsibilities.
I will be taking care of my grandfather who suffered from stroke
in Iloilo. I will be leaving on October 7. I will be resuming my duty
afterwards.
