resident of S.C. Colony, Singannapalem Village, Mundlamur Mandal,

Prakasam District, do hereby solemnly affirm and sincerely state as follows:-

I am the 3rd party herein in the above O.P. I know the above petitioners.
The 1st petitioners husband died in a motor accident and they filed the above
O.P. claiming compensation of Rs.6,00,000/- from the respondents 1 to 3, who
are the Driver, Owner and Insurer of the Lorry.

The petitioners 2 to 4 are

being minors represented by their mother, 1st petitioner as natural guardian.

The welfare and custody of the minor petitioners have been conducting by
their mother, 1st petitioner. There are no any disputes between the mother and
minor petitioners.
