Nursing Care Plan (Individual Assignment)

Scenario
Mrs. Wong aged 68 years old, lives alone and daughter visits once a week at home. She is admitted to
the ward with medical diagnosis Upper Respiratory Tract Infection. She has fever at home this
morning. The daughter reports that her mother was sneezing frequently and the amount of nasal
discharge is increasing. She told the nurse that her mother has hearing problems.
Past Health History
Ex-smoker, Hypertension
Nursing assessment
Mrs. Wong has fever with oral temperature 40C; normal blood pressure, dyspnea, rapid pulse 100/min,
body weight 55 kg. Skin is warm to touch and pink in color. Both ears are weak in hearing. A hearing aid
is needed on the left ear.
During hospitalization, Mrs. Wong has frequent productive cough, moderate amount of thick yellowish
sputum, malaise, loss appetite and she uses accessory muscles to breathe when sitting on a bedside
chair.
Mrs. Wong speaks Hakka language and needs her daughters assistance in translation during interview.
Her appetite is poor this morning. She is able to perform activity of daily living independently. Mrs.
Wong said I have headache, sore throat and experienced difficulty in breathing this morning. My
body aches started yesterday, I am weak and tired now.
Medical management
Diet as tolerated
Encourage fluid intake
Antibiotics as prescribed
Oxygen therapy

Students Instruction
Based on the scenario, students are required to search information, identify two appropriate nursing
diagnoses and develop a nursing care plan for Mrs. Wong. Word limit 1000 words.
Nursing care plan format: a nursing care plan template is posted on Moodle, word font size 12, Times
New Roman style, 1.5 line space. Each nursing diagnosis is presented on one care plan table, totally two
separate tables for two nursing diagnoses.
Deadline of assignment: students are required to submit individual work in the assignment box in 3rd
floor William MW Mong Block (date of submission: refer to course manual).
