PERMISSION LETTER FOR PHOTOGRAPHS AND VIDEOTAPES

Dear Parent/Guardian,
Hello! My name is Claudia Mesnil.
I am a student teacher from The University of Alabama in Huntsville.
During the next eight weeks, I will be working with Mr. White.
I am learning how to be teacher and I am very excited to be working at Ridgecrest
Elementary!
I will be taking pictures and/or videos of the classroom activities to show what we did
during my eight weeks at Ridgecrest. I must keep an electronic portfolio to with these
pictures and/or videos to graduate from the program.
I would appreciate your permission to use items that may include your child.
Everything will remain private and all pictures will remain my personal property only to
be used for educational reasons, none if it will be public or published.
Please check the relevant statement below, sign, and return the letter to Mrs. Mesnil.
_____I grant permission for my child to be photographed and/or videotaped for
educational purposes and for the photographs to be included in the candidates
electronic teaching portfolio. I understand that the photos, which will be used for
educational purposes only, will be posted on the UAH Department of Educations
website. Access to the candidates electronic portfolio will limited to the UAH campus.
