Dear Maams:
Good day!
This is to signify my consent that my son, undertakes his practicum at the Mother
Medical Center. I understand that this hospital is far from our residence but I truly
believe that the objective of an On The Job Training with this institution shall enrich
my sons learning and work experience. As such, please be advised that I am well
aware of the distance as well as the cost of a daily commute necessary for my son
to finish a 29 hour practicum requirement.
