AFFIDAVIT
We, RESALYN M. ADORIO , of legal age, Filipino, married and resident of
Bacolod City, respectively, having been duly sworn in accordance with law, hereby
depose and state:
1. That I am the secretary of the (SCALA) of the Immaculate Conception Parish
located at Brgy. Villamonte, Bacolod City;
2. That ARTURO T. FLORES and BELEN B. VERONICA, were lawfully
united at the Immaculate Conception Church of Brgy. Villamonte, bacolod
City, on June 03, 1977 and was solemnized by a Parish Priest of the said
Church;
3. That the Marriage Contract of the Spouses ARTURO T. FLORES and BELEN
B. VERONICA, which was for submitted to the Office of the Civil Registrar
of Bacolod City has not been submitted to the said office during the
prescribed period, because according to the previous secretary every time she
reminds the said priest to have the document notarized, he always said he will
be the one to facilitate it;
4. That however, the same was not transmitted to the Local Civil Registrars
Office until the present time;
5. That I am executing this Affidavit certify in order to establish the fact of the
delay in transmitting the marriage Contract of the above mentioned spouses in
the office of the City Civil Registrar of Bacolod City, for the reasons above
stated.
IN WITNESS WHEREOF, we have set our hands this 4th day of October ,
2010, at Bacolod City, Philippines.
