Saya dengan ini memohon dan membenarkan Takaful Ikhlas Berhad mengenakan kepada dan mendebitkan akaun kad
kredit saya untuk pembayaran bagi caruman bagi sijil-sijil Takaful seperti dinyatakan di bawah dan berikutnya berdasarkan
kepada jumlah caruman yang berkaitan (jika berkenaan).
I hereby request and authorise Takaful Ikhlas Berhad to charge to and to debit my Credit Card Account to pay the
contribution for the following Takaful certificates as stated below, and subsequently in accordance to the respective
contribution schedules (where applicable).

Term & Conditions of

TAKAFUL IKHLAS Credit Card Auto Debit

Sebagai pertimbangan kepada Takaful Ikhlas Berhad (selepas ini

dipanggil Syarikat) bersetuju untuk menerima pemberian kuasa ini,
saya/kami bersetuju dengan syarat-syarat berikut :-

In consideration of Takaful Ikhlas Berhad (herein after referred as the

Company) agreeing to accept this authorisation. I/We agree to the
following terms and conditions :-

1.

Saya/Kami akan menerima tanggungjawab sepenuhnya untuk segala

urusniaga melalui penggunaan kad kredit ini dalam pembayaran
caruman-caruman.
2. Syarikat tidak bertanggungjawab atau menanggung sebarang
tuntutan, kehilangan, kerosakan, kos dan perbelanjaan berpunca
dari pemprosesan debit, sama ada berjaya atau tidak, akibat
melebihi had kredit, pincang tugas sistem, kerosakan akibat sistem
elektrik terputus dan/atau faktor-faktor lain di luar kawalan Syarikat.
3. Syarikat hanya bertanggungjawab mengatur atau mendebitkan
akaun kad kredit saya/kami melalui Pusat Kad yang diarah oleh
saya/kami. Saya/kami mengesahkan dan bersetuju bahawa Takaful
IKHLAS Auto Debit Kad Kredit adalah disediakan oleh Syarikat
hanya semata-mata untuk kemudahan dan faedah saya/kami. Oleh
demikian, untuk sebarang masalah dan pertikaian yang timbul akibat
pemprosesan/pendebitan tersebut, adalah menjadi tanggungjawab
saya/kami untuk menyelesaikannya dengan syarikat kad kredit
(termasuk tetapi tidak terhad kepada masalah disebabkan oleh
kerosakan atau pincang tugas atau kerosakan mekanikal sistem
komputer atau kelengkapan kad kredit).
4. Saya/Kami dengan ini bersetuju untuk bersama-sama dan secara
berasingan melindungi Syarikat sepenuhnya atas sebarang tuntutan,
kehilangan, kerosakan, kos dan perbelanjaan yang Syarikat mungkin
alami atau tanggung berikutan daripada kebenaran saya/kami untuk
mendebitkan akaun kad kredit saya/kami seperti yang dinyatakan.
5. Pembayaran caruman akan diambilkira sebagai berbayar hanya
setelah pemprosesan debit yang berjaya oleh syarikat kad kredit,
dan pembayaran caruman diterima sepenuhnya, oleh Syarikat.
6. Saya/kami akan memastikan Syarikat diberi notis pemberitahuan
dalam bentuk bertulis atas sebarang perubahan, kehilangan atau
penggantian kad kredit saya/kami, atau pembatalan arahan
sekurang-kurangnya sebulan sebelum tarikh caruman berikutnya.
Perubahan sedemikian atau pembatalan akan berkuatkuasa hanya
selepas Syarikat telah memberi pengesahan penerimaan notis
pemberitahuan tersebut.
7. Syarikat berhak untuk menamatkan Takaful IKHLAS Auto Debit Kad
Kredit ini pada bila-bila masa tanpa memberikan sebarang alasan
dengan memberikan pemegang sijil notis pemberitahuan secara
bertulis.
8. Pembatalan Untuk memberhentikan Takaful IKHLAS Auto Debit
Kad Kredit dan menamatkan kebenaran ini, Pemegang Sijil atau
Pemegang Kad Kredit mesti memaklumkan kepada Syarikat secara
bertulis sekurang-kurangnya sebulan dari tarikh pemotongan untuk
pembayaran caruman yang seterusnya.
9. Resit Resit tidak akan dikeluarkan untuk pembayaran caruman
yang dibuat melalui Takaful IKHLAS Auto Debit Kad Kredit.
Saya/Kami akan merujuk kepada penyata kad kredit saya/kami bagi
pengesahan pembayaran.
10. Syarikat akan berusaha memotong jumlah penuh untuk pembayaran
caruman. Tiada bayaran untuk sebahagian daripada jumlah penuh
caruman boleh dibuat untuk bayaran caruman.
11. Di dalam keadaan di mana wang dikenakan atas atau didebit dari
akaun kad kredit di atas sebarang sebab, Syarikat dibenarkan untuk
mengembalikan wang tersebut sama ada kepada Pemegang Sijil
atau Pemegang Kad Kredit dan seterusnya bebas sepenuhnya dari
sebarang kewajipan berkenaan perkara tersebut. Sekiranya terdapat
sebarang pertikaian atau isu yang timbul dari pembayaran atau
pengembalian wang yang dibayar berikutan kebenaran ini, keduadua Pemegang Sijil atau Pemegang Kad Kredit akan merujuk hanya
kepada satu sama lain untuk pemulihan dan penyelesaian.

I/We shall accept full responsibility for all transactions arising

from the use of this credit in payment or contribution(s).
The Company shall not be held responsible or liable for any
claims, loss, damage, costs and expenses arising from the
successful processing of the debit due to exceeding credit
limit, malfunction of the system, electricity failure and/or any
other factors beyond the control of the Company.
The Company is only responsible for making arrangements to
debit my/our credit card account through the Card Centre as
authorised by me/us. I/We acknowledge and agree that the
Takaful IKHLAS Credit Card Auto Debit service is provided by
the Company solely for my/our convenience and benefit.
Therefore, for any problem or dispute arising form the
processing/debiting it will be my/our own responsibility to
resolve it with my/our credit card company (including but not
limited to any problems due to a breakdown or malfunction or
mechanical defect of the computer system or equipment of the
credit card company.
I/We hereby agree to jointly and severally indemnify and keep
the Company indemnified in full against any claims, loss,
damage, costs, and expenses which the Company may suffer
or incur arising from my/our authorisation to debit my/our
credit card account as aforesaid.
Contribution payment will be considered as paid only upon
successful processing of the debit by the credit card company,
and the actual receipt of the full contribution payment by the
Company.
I/We will ensure that the Company is notified in writing of any
changes, loss or replacement of my/our credit card, or
cancellation of this authorisation at least one month before the
next contribution due. Such changes or cancellation will
become effective only after the Company has duly
acknowledged receipt of such notification.
The Company may at its sole and absolute discretion
terminate this Takaful IKHLAS Credit Card Auto Debit service
at any time without assigning any reason by giving the
certificate owner a notice in writing.
Cancellation to discontinue the Takaful IKHLAS Credit
Card Auto Debit service and terminate this authorisation, the
Certificate Owner or the Credit Card Holder must inform the
Company in writing at least one month from the deduction
date for the next contribution due date.
Receipts Receipts will not be issued for contribution
payments made through the Takaful IKHLAS Credit card Auto
Debit service. I/We shall refer to my/our credit card statement
for confirmation of payments.
The Company will initiate an attempt to deduct the total sum of
contribution payment. No partial deduction will be made in any
instance.
In the event that any money charged to or debited from the
above credit card account is refundable by the Company for
any reason, the Company is authorised to refund the same to
either to Certificate Owner or the Credit Card holder and shall
thereafter be fully discharged from all obligations pertaining to
the same. Should any dispute or issue arise regarding any
payment or refund of moneys paid pursuant to this
authorisation, both the Certificate Owner and Credit Card
Holder shall refer only to each other for remedies and
resolutions.
