I, the undersigned, have completed BBA from North South University in May 2015. I need my
original certificate as I am applying for higher studies in USA. I am not in a condition to come to
your office in person to collect the certificate and hence I hereby authorize my mother, Shamina
Sharmin, to act in all matters necessary with regards to collect my original certificate. I ensure
you of my full consent with regards to this authorization. I am also attesting my mothers
