AMC Cares Scholarship - Congratulations!

Dear AMC Cares Scholarship Applicant,
Congratulations! It is a pleasure to inform you that you have been selected to receive a scholarship
from the AMC Cares Scholarship program for the 2014-2015 school year. The Scholarship Review
Committee consisted of Tulsa Community Foundation staff and volunteers who applaud your hard
work and dedication to achieving your educational goals. The AMC Cares Scholarship, a component
Fund of Tulsa Community Foundation, is available to AMC field associates that are not in a
management position and dependents of General Managers and Theatre Support Center associates
who are pursuing a degree in higher education.

This grant may be considered taxable by the IRS; check with a tax expert and/or your financial aid
office for more information. Your scholarship amount will be mailed directly to the institution of higher
learning that you will attend. It may be used for tuition, fees, books, or room and board. The
scholarship is a grant, and as such, does not have to be repaid. This is a one-time grant. You may
apply again next year if you still meet the eligibility requirements.
IMPORTANT: Responsibilities of Scholarship Recipient;
Complete the Student Agreement Form by clicking this link. Select AMC Cares Scholarship
from the Select your Scholarship Program drop down. This scholarship cannot be paid unless
and until you complete the Student Agreement Form.
You must submit to Tulsa Community Foundation your Fall 2014 course schedule which
includes the total number of credits you are enrolled in along with a University Bursar
statement.
At mid-year, you must submit to Tulsa Community Foundation your fall academic transcript
that includes all courses, grades, and a cumulative GPA along with your Spring course schedule
which includes the total number of credits you are enrolled in.
If you are not considered a student in good standing at your school, then you may no longer
eligible to receive this grant.
