IMPELLA -> intravascular micro axial blood pump

Inserted via femoral or axillary arteries descending aorta LV

Inlet in mid ventricular space and outlet in ascending aorta
Pulls blood from ventricle into aorta to support patient
hemodynamically by increasing cardiac output
Pulls blood through ventricle aortic valve outflow aortic
root
^^ AOP and flow and decreases EDV & EDP
rotation speed faster rotation = higher flow
Impella can operate on battery for 60 mins
Placement screen: placement signal and motor pressure help
determine where catheter is in respect to aortic valve
Purge flow regulated to keep pressure at 600 mmHg
Purge system delivers rinsing fluid (purge fluid) to prevent
blood from entering the impella catheter motor
o Pressure barrier from purge fluid flowing in the opposite
direction of blood flow
o Purge pressure must always be greater than systolic blood
pressure
o Purge fluid 5 40 % dextrose in water (20 % dex recc)
prevents blood from entering motor rinsing action
o If blood gets into motor gap, heparin maintains patency of
purge pathway
Placement signal monitor impella in correct position across
aortic valve based don physiologic pressure (BP)
CORRECT placement is aortic and motor is pulsatile
