AFFIDAVIT OF LOSS
I, IRIS JENNIFER JO, of legal age, Filipino citizen, with postal
address at No. 661 Craig St., cor. Fajardo St., Sampaloc, Manila, after having
been duly sworn to in accordance with law, do hereby depose and state,
that:
1.

I am a professional teacher with License No. 1068290;

2.

Sometime August 2015, while I was within the vicinity of

Makati City, I noticed that my wallet was missing;

3.

The said wallet contains all my personal identifications

including my professional identification card issued by the
Professional Regulation Commission;

4.

I religiously and patiently look for my missing professional

identification card but much to my effort I failed. The same is now
considered lost and beyond recovery;

5.

I am executing this affidavit to attest to the truth of the

foregoing statements, and in whatever legal purpose this affidavit
may serve;

6.

Further Affiant Sayeth Naught.
