address at No. 88 Litlit, Silang, Cavite, after having been duly sworn to in
accordance with law, do hereby depose and state, that:

I am a salesperson accredited by the Professional Regulation

Commission on 17 April 2015;

I was under the supervision of real estate broker Mr. Alfred

Ruther Borromeo Agustin with License No. 0018927 and registered
on 1 April 2014;

3.

Sometime January 2016, I was transferred to real estate

broker Ms. Marvie Mae Castillo with License No. 0025309 and
registered on 3 July 2015;

4.

I have no financial accountability from my previous real

estate broker;

5.

I am executing this affidavit to attest to the truth of the

foregoing statements, and in whatever legal purpose this affidavit
may serve;

6.

Further Affiant Sayeth Naught.
