Prerequisite: All girls participating on the spring iinterscholastic team must turn in a completed PIAA pre-
Participation physical and be cleared to participate. Forms are available on the Athietics page of the CCS
website,
Objective: Adhering to the Collegium Eligibility Policy, players will maintain grades, attendance and discipline
standards as we work to develop each athlete's skills while developing team building and good sportsmanship
traits to form a cohesive team that works together representing CCS with pride to compete against local public
‘and private high schools.
Location: Tryouts and practices will be held on the CCS campus. The team may be running throughout the
Oaklands Corporate Center as @ group during tryouts/practice. There may be times when the students are not
directly supervised by a CCS coach. ‘Students will be paired with a running partner. All home games will be
played at United Sports Center located at 1426 Marshallton-Thorndale Rd. Downingtown, Pa 19335.
Dates and Times: Open Gym: Tues, 3/1/16 through Thurs, 3/3/16 from 3:15 ~ 5:00pm. Tryouts: Monday, 3/7/16
from 3:15 — 6:15pm at CCS. Practice will begin Tuesday, 3/8/16 and end on Friday, 5/27/16. Parents are an
integral component to the success of the season, and your punctual arrival at the end of practices/games helps
keep the season moving smoothly,
‘Transportation: As a team member, athletes MUST ride the CCS bus to and from all home and away games.
Uniform: Come to tryouts and practice with lax stick, mouth guard, goggles, and water bottle. Uniforms are
provided and expected to be cared for. If any portion of the uniform is lost or damaged, the student athlete is
Fequired to pay for the replacement cost of the entire uniform. Washed uniforms are due back by Friday, May 27,
2016. There will be a $50 late fee for any uniforms turned in after the due date,
Participation Fee: Submit after tryouts, $25.00. Please make your check or money order payable to CCS.
Due Dates: Return completed permission and PIAA forms prior to first day of Open Gym, 3/1/16, and the
Participation fee by 3/11/16 if selected for the team
Refunds: There are no refunds,
Medical Concerns: If your child requires any medications during the activity, please contact CCS's school
nurse to make the necessary arrangements. Any health emergencies will be taken to the nearest emergency
room and parents will be contacted as soon as possible,
Registration: The permission slip below must be completed and is mandatory for participation,
