I, Angelisa G. Chavez of BS Architecture 3-3 would like to request at your office to
please allow me to take the Departmental Exam in our subject Building Technology
4 under Ar. Anthony Yan. I was not able to take the examination due to the conflict
with the schedule we had in our CAD class last February 20, 2016. I did not intended
to skip the said examination. I fully regret what happened and I am hoping for your
kind consideration.
