Re: Denise Regina Madden
Dear Dr. Cooper:
Thank you for referring Ms. Madden to me.
Denise was complaining of anterior chest pain, pleuritic in nature, that had
been aggravated by lifting or using her upper extremities. She also has had
problems for the past several weeks with pain in her left hip, aggravated by
activity.
My examination revealed tenderness over the left greater trochanter with
positive Patricks test; otherwise the exam was negative except for the chest
wall, where there is tenderness along the costochondral margins bilaterally.
Heart sounds are normal. Abdomen is benign.
My diagnoses are costochondritis and left greater trochanteric bursitis.
The left hip was injected with 1.5 mL of triamcinolone suspension in 3.5 mL
Marcaine with good partial relief of symptoms. She was placed on Naprosyn
and exercises for the hip. I will follow up with her if she does not show
improvement.
