To whom it may concern,

I would like to apologize for not being able to validate my school ID for the
reason that I have misplaced my registration form. I have no idea that I can request
for another copy of it since I am a transferee student. Last December, Ive asked
some of my classmates and theyve told me what to do. But then, I became too
busy preparing and studying for the Preliminary Exam. Thank you for your kind
