Welcome to DOT Family Health Center. We are proud to announce that DOT Foods has
chosen us as their on-site medical clinic to provide you and your dependents primary
health care, and we are eager to be of service. We want to help shape the culture of health
for you and your family through our innovative and affordable health care services. Enjoy
reduced co-pays on your labs, office visits, and medications, more time with our providers,
and wellness coaching on a variety of health topics.
Your on-site Family Health Center is located at 1534 North State Road 1, Cambridge City,
IN 47327. Patients are seen by scheduled appointment only, though we offer many same
day appointments for acute needs.
Our patient call center is open Monday through Friday from 8:00 AM to 5:00 PM, Eastern
Standard Time. Please call 1-800-920-4185 ext 89 to schedule an appointment.
When you arrive for your first appointment, please bring the following items with you:
1. All of your health insurance cards
2. Photo identification
3. A list of all medications you are currently taking, including vitamins and over the
counter medications.
You will be asked to fill out a new registration form so we ask that you allow plenty of time
to get to the office for your appointment.
