Affidavit of Accident

I, __________________, the authorized driver of __________________, with plate

number ______________, was driving along ___________________ on
_________________, when a truck hit the back of my car as it was turning right
at the corner of ___________________. The truck just drove away without
stopping to check the accident and I was not able to catch up on him nor get
its plate number. The incident resulted to the _________________broken tail
light and two deep and long scratches on the right side.
