Applicant clicks SUBMIT after filing out the mandatory fields. A CONFIRMATION will appear if registration was successful. If not, the process should be repeated 3. Applicant clicks the PRINT CONFIRMATION LETTER button flashed on the screen to print his/her confirmation letter. NOTE: The Examination Center where the applicant Shall take the Entrance Examination, is dutomatically assigned by the computer system based on the permanentthome address provided by the applicant. IMPORTANT ‘Applicant must be at the NAPOLCOM Regional Office on his/her scheduled appointment date with all the requirements. VI. WHERE TO GET APPLICATION FORMS AND INDEX CARDS Application Forms (NAPOLCOM Form 1-A) and Index Cards can be obtained for FREE at any NAPOLCOM Regional Office nationwide, or may be downloaded from the NAPOLCOM website at wwnw.napolcom.gov.ph. These may also be REPRODUCED using the original size of the Application Form and Index Card. 4:Vil vil. HOW TO FILE ‘An applicant should file his/her application, together with all the required supporting documents, on his/her scheduled appointment PERSONALLY only at the NAPOLCOM Regional Office where he/she will take the examination. APPLICATIONS RECEIVED THRU POSTAL MAIL SHALL NOT BE PROCESSED. WHAT TO BRING WHEN FILING 1. Printed letter-reply with corresponding reference number confirming the scheduled appointment 2. An Application Form (NAPOLCOM Form 1-A) and Index Card, duly accomplished by the applicant and two (2) recent and identical 1" x 1” ID pictures (in color) with white background and complete name tag (indicate rank before the first, middle and last names, if PNP member) 3. Police Examination Fee: Four Hundred Pesos (P400.00) _ i. = NO OTHER FEES SHALL BE COLLECTED. One (1) legal-size window envelope with Seventeen’ Pesos (P17.00) worth of mailing stamps. Transcript of Scholastic Record (with Special Order issued by CHEd) and Diploma from CHEd-accredited schools, state universities and colleges. 6. Birth Certificate (BC) in Security Paper issued by the Philippine Statistics Authority (PSA) or Certified True Gopy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. 7. For a PO with temporary appointment, a certified true copy of Attested Appointment, Plantila Appointment or Special Order. os ‘An examinee who fails in an exam or after being admitted to take said exam, fails to take it for one ‘eason or another, need not resubmit the requirements should he/she apply again for the same exam in the same exam center. NOTIFICATION OF RESULTS: ‘The nationwide List of Successful Examinees will be posted at the NAPOLCOM Central Office located at DILG-NAPOLCOM Center, NAPOLCOM Building, EDSA corner Quezon Avenue, West Triangle, Quezon City 1104 and at the NAPOLCOM website. Similarly, a Regional List will be posted in each of the NAPOLCOM Regional Offices. For inquiries, examinees may log on to the NAPOLCOM web address at www.napolcom.gov.oh EXAMINATION RESULTS - Report of Rating (RR) will be mailed to the examinee's mailing address. Verification may be made with the Examination Division, Personnel and Administrative Service (PAS), NAPOLCOM Central Office. An examinee who passed the test but did not receive hisfher RR may request for the issuance of a certification from the same office, upon payment of ‘One Hundred Fifty Pesos (P150.00).
