Materials:
A piece of paper labeled Check in and Check out and a pen or pencil

Procedures:
Students will report to the main office where they will write down the time they arrived at
school every morning before reporting to class, this will be checking in. After school is
over they will report once again to the main office and write down the time they are
leaving, this will be checking out. This will happen monday through Friday. If a student
makes it through the week without any absences he or she will get a point of extra credit.
Plan for Evaluation: How will each of the following be collected?
Process Data:
4 6th grade students will participate in this activity throughout the whole school year
Perception Data:
Pre and Post test will be administered
Outcome Data:
75% of students will be able to decrease the number of absences
75% of students will be able to identify the reasons why they were not coming school
Follow Up:
We will follow up with students at the end of the month to see how many absences they
have and what was the reason
