The Excel series features a wide choice of anesthe- sia delivery options: Anesthesia * The Excel 210 offers three gases, oxygen, nitrous oxide, and air or éehvery heliox (75% helium, 25% oxygen). All gases except heliox (cylinder only) have pipeline and cylinder connections. * The Excel 110 has oxygen and nitrous oxide. All gases have pipeline and cylinder connections, with a backup cylinder connection for one gas, + Both models hold two vaporizers (Tec 4 or 6). Ventilator / ‘There are two basic Excel monitoring/ventilation packages: monitoring packages ‘+ The monitor pod and the Ohmeda 7000 Ventilator (a basic electronic ventilator). The monitor pod features panel mounted versions of the stand-alone oxygen, airway pressure and volume monitors, Volume and airway pressure monitors are not standard on Canadian monitor pods. ‘+ The Ohmeda 7800 Ventilator. The 7800 Ventilator combines ventila- tor and monitoring functions and offers a broader range of ventilation capabilities. + The Ohmeda 6250 Respiratory Gas Monitor (RGM) works with both packages. Standard RGM capabilities include O concentration, airway pressure, COz and N20 monitoring. Options include airway flow, SpO2, anesthetic agent, and circuit Oz monitoring, Safety features ‘The Excel includes several important safety features: + The Onmeda Link 26 Proportion Limiting Control System limits the lowest oxygen concentration that can be delivered on the Excel to a nominal 25% for 0,/N,O mixtures at the common gas outlet + Audible and visual low O, supply alarms alert you if the O, pressure falls below anominal 30 psig (207 kPa). + _N,O and third gas flows stop if 0, supply pressure falls below a nominal 20 psig (138 kPa). ‘+ An interlock mechanism that helps prevent more than one Ohmeda ‘Tec 4 or Tec 5 vaporizer from being “On” at once. An isolation sys- tem also helps prevent fresh gas from entering a vaporizer that is not switched “On.” + Power failure alarms and monitoring backup. * Differentiated pipeline and cylinder gauges, * Gas specific pipeline and cylinder connections. + A guarded O, flush button. + Color coded flow controls and pressure gauges. serene ee Ve 1/Introduction Operator For your convenience, the Excel also features: ‘+ Assystom master switch that controls the pneumatics and the elec- tronics. + Asstandard two drawer storage cabinet. ‘+ An 0; power outlet, typically connected to a ventilator. + Pull length dovetail accessory mounting. + Assingle action brake/footrest. + Shoif space for additional monioring. + Large, easy-tunning casters and a compact frame. How to use this manual wnat the manuals symbols mean ‘This manual gives you an overview of the available Excel Systems with ‘emphasis on the gas machine. Each system component also has an ‘operation and maintenance manval to answer specific questions. A complete sot of operation and maintenance manuals comes with the ‘Excel System. Onmeda recommends ths: you keep this manual and all elated manuals in one of the storage cabinet drawers. If you are using the system for the first time, read this manual first. Thea. ead the operation and maintenance manuals for the system compo- nants, Before you go on to Chapter 2, "General Information,” make sure that you understand the symbols listed at the end of this chapter. If you have used the Excel before, complete the step-by-step set up ‘procedure in Chapter 3. Then turn to Chapter 4 “Preoperative Checkout, ‘which is required before using the Excel Before cleaning the Bxcel, read Chapter 5, “Maintenance,” carefully. This section also tells you how to replace customer accessible fuses, reset circuit breakers, and replace complete monitors. Chapter 6, “Troubleshooting,” helps you solve problems that may occur with the Excel. For example, how can you tell if a low pressure leak is due to a vaporizer that you can replace or an intemal fault that requires a service call? (If the leak follows a particular vaporizer and the external o- fag is in place, replace the vaporizer). Chapter 7, “Illustrated Parts,” tells you Low to order replacement parts and accessories. Figures 2-1 through 2- 4 show both Exeal Systems. ‘The type of ventilator determines how the Excel System functions. "7800 Ventilator ‘Systems with the 7800 Ventilator: + Use oxygen, volume and pressure monitors built into the ventilator. * Distribute electrical power through an outlet box. Electrical power cords for system components plug into auxiliary outlets on the box. A transformer inside the outlet box supplies power to the light pack- age + Use the system master switch to control gas flow and to switch “On” the ventilator. + Display ventilator and respiratory gas data together on the optional Respiratory Gas Monitor (RGM). 7000 or non-Ohmeda ‘Systems with the 7000 Ventilator or a non-Ohmeda ventilator: rentilator + Use separate oxygen, volume and pressure monitors mounted in the monitor pod. The interface panei on the monitor pod allows sensor connections. Volume and airway pressure monitors are not standard on Canadian monitor pods. ‘Switches the light package “On” and “Off.” With a monitor pod, the light package transformer must be plugged into an outle: before it has power. With the system master switch set to “On,” tuming a fow control counter-clockwise increases flow, turning a flow control clockwise decreases flow. A linkage between the N,O and the Q, flow controls limite the lowest oxygen concentration that can be set on the Excel toa nominal 25% for 0,/N,O mixtures at the common gas outlet. Flow ranges are: O,, 200 ml/min to 10 I/min {plus up to one additional turn of the knob in Canada, two in the United States and elsewhere); N,O, 0 to 10 l/min (plus up to one additional tum of the imnob }; sir or heliox: 0 to 16 /min (plus up to one additional tum of the knoa). Pneumatic indicator toggles to red if O, pressure drops below approxi mately 30 psig (207 kPa). Setting the system master switch to “On” allows gas to flow through the Excel and activates the electrical system, either switching “On” the 7800 ‘Ventilator or the monitor pod. The locking lever must be fully clockwise (locked position) or the control will not turn. Pressing the release allows you to turn the concentration knob on the vaporizer. ‘When the locking lever is completely counter-clockwise, and the vapor- izer concentration js set to “Off,” you can mount or remove the vaporizer. Turn the lever clocwise to lock the vaporizer in position. Opens the fil port valve so that agent can be added to the vaporizer. Folds the fill adapter from the agent hottle in place when the vaporizer is boing filled. To secure the adapter, lower the lover, insert the adapter, and raise the lever, Unscrew and remove the plug to fill or drain vaporizers with funnel fl ports. Insert the adapter from the agent bottle :o fill or drain the vaporizer. If power fails, you must set the switches so the monitors use thelr internal batter- ies (see chart), After power is restored, you must return the oxygen and airway pressure monitor switches to the original settings or the monitors ‘will continue to run off internal batteries. A fully charged battery can run the alarm LED and speaker for 1.25 hours at the normal volume and brightness and at reduced levels for an additional 45 minutes. To re- charge the battery, set the Excel system master switch to “Off” and plug the monitor pod electrical power cord into an energized electrical outlet. Recharging takes approximately 14 hours. A rachargeable battery inside the 7000 Ventilator runs an audible power failure alarm. To recharge the battery, set the 7000 Ventilator power switch to “On” and plug the ventilator's electrical power cord into an ‘ene:gized electrical outlet. Recharging takes less than 21 hours. 210 2/General Information 7000 Ventilator Note: Refer to the 7000 Ventilator Operation and Maintenance Manual for detailed information. ‘The 7000 Ventilator has no display or patient monitoring. It has @ non adjustable high pressure relief valve and alarms for ventilator malfunction, control settings that exceed unit capabilities (set volume not delivered, actual less than control setting), low airway pressure, and low O, supply pressure. An internal battery powers an audible power failure alarm. There are some differences in monitoring depending on ‘whether or not the ventilator is set for mechanical ventilation. The ventilator battery is rechargeable. To recharge the battery, set the Excel system master switch to “Off” and plug the ventilator's electrical power cord into an energized electrical outlet. Set the syscom master switch to “Off.” 2. Swing out the yoke clamp. 3. Unscrew the tee handle until the screw is flush with the inside of the clamp. 4. Make sure that the cylinder valve is not covered by a dust cap. Remove dust cap if present, 5. Remove the old cylinder gasket if present and install a new gasket. ‘Make sure that the old gasket does not stick to the cylinder. CAUTION: Use one cylinder gasket per yoke. Extra gaskets may cause a leak. WARNING: Always close the cylinder valve when you are using a pipeline supply. If a cylinder valve is open and the pipeline and regu- lated cylinder prossures are equal, both supplies may be w neously, leaving an insufficient reserve in case of pipeline failure. Circuit and monitoring connections A Note: Use this section as a quick reference if you are alroacy familiar with individval monicer, ventilator, and absorber connecticns. For cerallad information and accessory part numbers, refer to the individual operation and maintenance manual. WARNING: Use only Ohmoda specified cables, hoses, and tubing for external connections. Alternative cablos, hoses, or tubing could cause false sensor readings or damage to the system. 1. Set the system master switch to "Olt". 2. Make sure that tae absorber and the ventilator bellows are securely mounted. 3. Make the monitoring connections: WARNING: To ensure accurate pressure monitoring, the pressure Sensing line must be Kept clear of condensate. Ifyou are using a non-Oameda ventilator that requires expiratory pressure monitoring, use the 7000 Ventilator connections. + Ifyou have a 7000 Ventilator and a GMS Absotber, refer to Figure 3-2. ‘+ Ifyou have a 7800 Ventilator and a GMS Absorber, refer to Figura 3-3 ‘+ Ifyou have a 7000 Ventilator and a non-GMS Absorber, rofer to Figure 2-4. Electrical connections A WARNING: To help protect against shock hazards, connect this equip- ment only to a grounded, hospital grade receptacle. The grounded plug must be inserted into a properly wired grounded receptacle. Where an ungrounded receptacle is encountered, a qualified electrician MUST replace it with a properly grounded receptacle. Do not use extension cords or adapters of any type. The power cord and plug must bo intact and undamaged. 1. Check electrical connections: With a 7800 Ventilator, make sure that the ventilator is plugged into an auxiliary outlet in the power outlet box and that the Excel/ventila~ tor interface cable is connected. With 2 7000 Ventilator, make sure that the ventilator is plugged into an auxiliary outlet, 2. Check that the system master switch is set to “Off.” Then, plug the Excel power cord into an appropriately rated electrical outlet. 42 4/Preoperative Checkout 3. If a ‘Tec 4 and a Tec 5 vaporizer are used together, the interlock system still helps prevent more than one vaporizer from being switched on at the same time. WARNING: Do not attempt to modify the system to accept Ohmeda Tec 3 vaporizers. The safety interlock will not prevent a mixture of, Ohmeda Tec 3 vaporizers with Ohmeda Tec 4 or Tec 5 vaporizers from ‘operating simultaneously. 1. Make sure that the tops of the vaporizers are parallel to the top edge of the manifold, Remount any vaporizers that are out of line. 2. Check the vaporizers' locking knobs to ensure that the vaporizers are locked in place. 3. Attempt to lift each vaporizer off of the manifold. Remount any vaporizer that is not securely locked in place, WARNING: Do not use a vaporizer that continuously lifts off the manifold when its locking lever is in the lock position. 4, Attempt to switch “On” more than one vaporizer at a time. Try every combination that is possible with your system, If moze than one vaporizer can be switched “On” at a time, remount the vaporizers and repeat this step. WARNING: Do not use the system if the vaporizer interlock system allows more than one vaporizer to be switched “On” at a time. 5, Fill the vaporizers as described in the vaporizer operation and mai tenance manual. ceoLono en 43 4/Preoperative Checkout Checking cylinder and pipeline supplies 1, With the cylinders closed, make sure the eylinder gauges read zero If the cylinder gauges do not ead z0r0, the cylinders may have boon previously opened. Briefly set the system master switch to "On" and ‘open the flow control valves to bleed pressure. As pressure de- creases, a "(ld SLA. PRES (” alarm may trigger on the 7800 Ventilator. A CAUTION: Open cylinder valves slowly to help avoid damaging the regulators. 2. Close all flow control valves and open the cylinders. Check that the cylinder gauges show adequate pressure. 3. Close all of the eylinder valves and note the value on each cylinder pressure gauge. The gauges must show less than a 100 psig (690 kPa) pressure drop ina five minute period. If the pressure drop excseds this limit, the high-pressure circuit has an unacceptable leak. i there is an a. Check for a defective cylinder gasket, an extra cylinder gasket, or uunacoeptable leak: a loose tee handle. Replace the gasket(s) with a single, undam- aged gasket or tighten the tee handle. b, Repeat the leak check. If the circuit still leaks, do not use the system for clinical applications. Return the system master switch to “Off.” Checking the gas flow controls A Note: WARNING: The Link 25 Proportion Limiting Control System sets a minimum O, concentration in the fresh gas stream when only 0, and N,0 are used. Recirculating through an absorber or using a third gas may still cause a hypoxic mixture to be delivered, especially at low 0, flow rates. 1, Either connect the pipeline supplies or slowly open the cylinder valves. 2. Set the system master switch to “On” and tur all flow control valves completely clockwise (minimum flow). With the 7800 Ventilator, status messages such as “REW FLOM ALM FF", “APHER ALARM OFF ", or *UOL MOM STANDBY * may appear. VENT FAIL XX " or "LOW BATTERY * alarms should not appear. Low minute volume alarms may still occur and can be silenced with the alarm silence button. 3. The oxygen flowmeter should show about 200 ml/min. The other flowmeters should show no gas flow. 4. Use the Link 25 Proportion Limiting Control System to check the N,O flow control. Refer to the bellows Sterilization Use this section as a quick reference once you are familiar with the sterilization procedures found in the individual operation and mainte- nance manuals. ‘These items cannot be sterilized: ventilator control module and moniter pod monitors. ‘These items can be sterilized with ethylene oxide: bellows assembly (disassembly required), volume sensor cartridge, front half of the oxygen sensor housing, oxygen sensor adapter, oxygen sensor cartridge (ambi- ent temperature, no vacuum) and rubber and plastic articles. Refer to individual operation and maintenance manuals for disassembly instruc- tions end additional information. ‘The volume sensor assembly can be sterilized with a disinfectant applied with a cloth (do not immerse). The volume sensor cartridge can also be sterilized with liquid agents, but it must be dried thoroughly before use. To avoid damaging the precision movement, do not drop the cartridge or use a cleaning brush. ‘The O, sensor cartridge and rubber and plastic articles can be sterilized with a cold germicidal detergent, Do not immerse the O; sensor cartridge. Special precautions for rubber articles Rubber goods deteriorate over time and are considered expendable. The presence of oxygen, ozone, ether, mineral or vegetable ails, phenols, cresols, terpenes, hydrocarbon solvents, chlorinazed hydrocathors, esters, or oxidizing agents will hasten deterioration, Check rubber articles regularly. Replace them when any of the following signs of deterioration appear, swelling, tackiness, or cracking. Conductive rubber goods loose their electrical conductivity with age. To extend the useful life of rubber articles: * Remove metal connectors immediately after use. ‘+ Store rubber articles in the dark away from ozone souress (florescent light fixtures, electric motors, and diathermy machines). WARNING: Do not use tale, zinc stearate, calcium carbonate, com. starch or similar substances to prevent tackiness on rubber articles. Most of the pneumatic circuitry is located undemeath the tabletop. ‘The Excel 110 comes with gas supply modules for nitrous oxide and oxy- gen. The Excel 210 adds a third gas, either air or heliox, Gas supply modules contain two parallel branches, pipeline and cylinder. ‘The pipeline supply branch consists of a filtered pipeline connection and a pipeline pressure gauge. The cylinder branch consists of a filtered cylinder connection, a cylinder gauge, and a high pressure gas regulator that re- duces cylinder pressure to a nominal 4§ psig (311 kPa). Waen only one source is connected or switched “On," check valves keep gas from leaking out through the unused branch, When both pipeline and cylinder supplies are connected, make sure that the cylinder valve is closed. Otherwise, cylinder gas could be depleted since the pipeline pressure is almost the same as the reculated cylinder pressure. A pressure relief valve in each gas, supply module limits maximum pressure to 75 psig (518 kPa), Shut-off valves control the flow of the nitrous oxide and the third gas. When the system master switch is set to "Off”, these valves are closed. When the ‘system master switch is set to “On,” the pilot oxygen pressure holds them open. As long as the pilot pressure remains above the critical level, approxi- mately 20 psig (138 kPa), the valves remain open. If the oxygen pilot pres- sure drops below this level, the saut-off valves close, stopping the flow of nitrous oxide and the third gas. You should notice the pilot oxygen flow through the oxygen flow tube whenever the system master switch is "On,* even if the oxygen flow control is cured completely clockwise (minimum flow). Secondary regulators reducs the oxygen and nitrous oxide pressures to the evels required by the flow controls. The third gas goes directly from the ‘primary regulator to the flow control. A gear linkage on the nitrous oxide and oxygen Sow control knobs helps limit the lowest O, flow control setting tc a nominal 25% of the N,O flow control setting. A system of mechanical stops sots the maximum flows. ‘The mixed gas output of the fowmeter assembly goes through the selected, vaporizer and into the patient circuit. A pressure relief valve on the common {gas outlet limits pressure to approximately 4 to 5.5 psig at 200 ml/min. This same warzanty is made for a period of thirty (30) days with respect to expendable parts. The foregoing warranties shall not apply if the Product, has been repaired other than by Ohmeda or in aecordance with weltten instructions provided by Ohmeda, or altered by anyone other than ‘Ohmeda, or if the Product has been subject to abuse, misuse, negligence, or accident, Ohmeda’s sole and exclusive obligation and Buyer's sole and exclusive remedy under the above warranties is limited to repairing or replacing, free of charge, at Ohmeda’s option, a Product, which is telephonically reported to the nearest Ohmeda Regional Service Office and which, if so advised by Ohmeda, is thereafter returned with a statement of the observed deficiency, not late: than seven (7) days after the expiration date of the applicable warranty, to the designated Ohmeda Service Center during normal business hours, transportation charges prepaid. and which, upon Ohmeda’s examination, is founc not to conform with above warranties.
